Comparing the Effects of Traditional Versus Simulator-Based Mechanical Ventilation Training on Nurses' Clinical Competence

Background & aim: Mechanical ventilation management is an essential clinical skill for nurses caring for critically ill patients; however, the most effective training approach for developing this competency remains unclear. This study compared the effects of mechanical ventilation training on nurses' clinical competence using traditional and simulator-based methods.
Methods: This quasi-experimental study was conducted during 2022–2023 at Baqiyatullah Hospital (AJ) in Tehran, Iran. A total of 60 nurses from critical care units were selected using convenience sampling and randomly assigned to two groups: traditional training (30 nurses) and simulator-based (30 nurses). The simulator-based group received training using a mechanical ventilator simulator. The traditional group received lecture-based instruction supported by PowerPoint presentations. Clinical competence was assessed before and two weeks after training using an Objective Structured Clinical Examination (OSCE) and a six-station checklist.
Results: At baseline, the two groups were comparable in demographic characteristics and clinical competence scores (P>0.05). Following the intervention, the simulator-based group demonstrated significantly greater improvement in clinical competence than the traditional group (P = 0.001). Finally, data analysis was performed using descriptive statistics (frequency, mean, and standard deviation) and inferential statistics (independent t-test, chi-square, and paired t-test).
Conclusion: According to the results of the present study, it is suggested to use new simulator-based educational approaches and to implement the concepts practically on the ventilator, in the clinical qualification in mechanical ventilation.
