Gangrene of All Four Limbs in a Patient with Pemphigus Vulgaris Following Norepinephrine Use; A Rare Complication: Case Report

Background & aim: The use of epinephrine in a patient with pemphigus vulgaris, characterized by flaccid and erosive blisters of the skin and mucous membranes, led to infection and gangrene of all four limbs.
Case Report: This report describes a rare case of gangrene affecting all four limbs in a 59-year- old woman with pemphigus vulgaris, a severe autoimmune blistering disorder, following the administration of norepinephrine. Pemphigus vulgaris is characterized by flaccid and erosive blisters on the skin and mucous membranes caused by autoantibody-mediated disruption of cell adhesion, leading to increased susceptibility to infection and reduced quality of life. Norepinephrine, a potent vasoconstrictor, can cause serious irreversible vascular complications in patients with underlying autoimmune conditions.
Purpose of the Report: The interplay between autoimmune pathophysiology and vasopressor therapy, in the management of complex autoimmune conditions highlights the need for heightened clinical vigilance and individualized treatment decisions.
