Background & aim: Patients with mechanical heart valves are required to receive lifelong anticoagulation. To standardize care, nurses can use nursing models to increase patient compliance. The aim of this study was to apply the Roy adaptation model to the care of a patient with a mechanical heart valve. Methods: This case study was conducted with the participation of a patient with a mechanical heart valve hospitalized at Farshchian Heart Hospital in Hamadanfor three monthsin 2024. Data were collected using observation, interview, and patient file documentation and were examined in four dimensions based on the stages of the Roy nursing process. Results The results showed that the patient had maladaptive behaviors in physiological dimensions, self-perception, role performance, independence, and dependence, and that after providing nursing care based on the Roy adaptation model, the patient's adaptive behaviors increased. Conclusion: Implementing a nursing care program based on the adaptation model can be effective in patients' adaptation to mechanical heart valve replacement. Accordingly, the use of this model is recommended for nurses.
