fa مراقبت انسان‌گرایانه در بخش مراقبت‌های ویژه: رویکردی فراتر از مراقبت بالینی در عصر تکنولوژی Humanistic Care in the Intensive Care Unit: An Approach Beyond Clinical Care in the Age of Technology تخصصي Special پژوهشي Research زمینه و هدف: بخش مراقبت‌های ویژه، یکی از پیچیده‌ترین و حساس‌ترین عرصه‌های نظام سلامت با پیچیده‌ترین بیماران است؛ بیماران بستری در این بخش با استرس‌های روحی و جسمی متعددی روبه‌رو هستند. در سال‌های اخیر، با وجود افزایش میزان بقای بیماران در بخش مراقبت‌های ویژه، برخی از ابعاد انسانی مراقبت مورد غفلت واقع شد‌ه‌است. فرایند مراقبت در طول زمان تغییر کرده و به مراقبت انسان‌گرایانه تکامل یافته‌است؛ در مراقبت انسان‌گرایانه تمامی ابعاد مراقبت و تمامی ذی‌نفعان در فرایند مراقبت در نظر گرفته می‌شوند. در عصری که فناوری‌های نوین و پیچیدگی مراقبت‌های بالینی به ‌سرعت در حال گسترش‌ هستند، حفظ و تقویت ارزش‌های انسانی در مراقبت‌های پرستاری بیش از پیش ضروری است؛ هدف از این مطالعه، معرفی مدل و چارچوبی برای ارائه مراقبت انسان‌گرایانه در بخش مراقبت‌های ویژه در عصر تکنولوژی حاضر است. روش‌ها: یکی از جامع‌ترین مدل‌ها به ‌منظور مراقبت انسان‌گرایانه در بخش مراقبت‌های ویژه، مدل HU-CI است که هفت بُعد دارد؛ این ابعاد شامل "آی‌سی‌یو با درهای باز (Open- Door ICU)"، "ارتباط مؤثر (Communication)"، "رفاه بیمار (Patient Well-Being)"، "مراقبت از کارکنان آی‌سی‌یو (Caring for Professionals)"، "سندرم پس از آی‌سی‌یو (Post-Intensive Care Unit Syndrome (PICS))"، "مراقبت‌های پایان زندگی (End-of-Life Care)" و "زیرساخت‌های انسان‌محور در آی‌سی‌یو (Humanized Infrastructure)" است. یافته‌ها: مدل حاضر با ادغام دانش علمی و ارزش‌های انسانی بر اصولی همچون شفقت، ارتباط مؤثر و مراقبت از تیم درمان تأکید دارد و اجرای آن باعث افزایش رضایت بیمار و خانواده، بهبود نتایج بالینی، ارتقای کیفیت مراقبت، همبستگی تیمی و ارتقای حرفه‌ پرستاری می‌شود. نتیجه‌گیری: مدل HU-CI با در نظرگرفتن زیرساخت‌های انسان‌محور به دنبال تعادل و تلفیق بین فناوری‌های پیشرفته و ارزش‌های انسانی و هم‌سوکردن مراقبت با عصر تکنولوژی است؛ این مدل با تأکید بر انسان‌محوری، همدلی و مراقبت معنادار، ظرفیتی ارزشمند برای تحول در مراقبت‌های ویژه فراهم می‌سازد و می‌تواند چشم‌اندازی نوین در مسیر توسعه حرفه‌ای پرستاری و پاسخ‌گویی به نیازهای انسانی بیماران در شرایط بحرانی ترسیم کند. Background & aim: The intensive care unit (ICU) is one of the most complex and sensitive areas of the health system, with the most complex patients; patients who are hospitalized in this unit encounter numerous mental and physical stresses. In recent years, despite an increase in inpatient survival rates within the ICU, some human dimensions of care have been neglected. The care process has changed over time and evolved into humanistic care, which considers all dimensions of care and all stakeholders in the care process. In an era where new technologies and the complexity of clinical care are rapidly expanding, preserving and strengthening human values in nursing care is more essential than ever; the aim of this study is to introduce a model and framework for providing humanistic care in the (ICU) in the current technological era. Methods: One of the most comprehensive models for humanistic care in the ICU is the HU-CI model, which has seven dimensions. These dimensions include "Open-door ICU", "Effective Communication", "Patient Well-being", "Caring for professionals", "Post-intensive care unit syndrome (PICS)", "End-of-life care" and "Humanized infrastructure in ICU". Results: By integrating scientific knowledge and human values, the present model emphasizes principles such as compassion, effective communication and care of the treatment team, and its implementation increases patient and family satisfaction, improves clinical outcomes, enhances quality of care, team cohesion and promotes the nursing profession. Conclusion: HU-CI model, by considering human-centered infrastructures, aims to balance and integrate advanced technologies and human values, and to align care with the technological age.
