fa ارتباط نمره آپاچی2 و پی اس آی با میزان مرگ ‌و میر و مدت بستری بیماران کووید-19 بستری در بخش ویژه: یک مطالعه مقطعی The Relationship between Apache 2 and PSI Scores with Length of Hospitalization and Mortality Rate of Covid-19 Patients Hospitalized in Intensive Care Unit: A Cross-Sectional Study تخصصي Special كاربردي Applicable <div aria-controls=":tw" aria-expanded="false" aria-label="Message Body" aria-multiline="true" aria-owns=":tw" class="Am aiL Al editable LW-avf tS-tW tS-tY" contenteditable="true" g_editable="true" id=":ri" itacorner="6,7:1,1,0,0" role="textbox" spellcheck="false" style="text-align: justify;" tabindex="1" writingsuggestions="false">زمینه و هدف: در بخش‌های مراقبت ویژه سیستم‌های متعددی جهت پیش‌بینی مرگ و میر بیماران به کار گرفته می‌شود. در این مطالعه ارتباط نمره آپاچی2 و پی اس آی با طول مدت بستری و میزان مرگ و میر بیماران کووید-19 بستری در بخش مراقبت ویژه بررسی‌شده است. روش‌ها: این مطالعه مقطعی تحلیلی، بر روی بیماران بستری در ICU انجام‌شده است. به‌ منظور جمع‌آوری اطلاعات از ابزار PSI و ابزار آپاچی2 استفاده شد، به این صورت که پس از اخذ معرفی‌نامه از معاونت پژوهشی دانشگاه به واحد بایگانی بیمارستان‌ها مراجعه شد، سپس داده‌های موردنیاز برای ابزار آپاچی2، ابزار PSI و مشخصات دموگرافیک مربوط به 24 ساعت اول پذیرش در ICU از پرونده‌ها و HIS بیمارستان جمع‌آوری شد. اطلاعات مربوط به زمان فوت، طول مدت بستری و انتقال بیمار نیز ثبت شد. یافته‌ها: از 486 بیمار ارزیابی‌شده، میانگین و انحراف معیار سن 18/1 ± 62/76 سال بوده، 46/7 درصد (227 نفر) از بیماران زن و 53/3 درصد (259 نفر) مرد بودند. میانگین مدت بستری در بخش مراقبت‌های ویژه 4/6 ± 7/15 روز (حداقل 1 حداکثر 27 روز) بود؛ 21 درصد (102 نفر) در زمان بررسی اینتوبه بودند؛ و نهایتاً 15/9 درصد (252 نفر) از بخش ترخیص شدند و 48/1 درصد (234 نفر) فوت کردند. در ارزیابی اخیر مشاهده شد که نمره آپاچی2 با مرگ و میر بیماران ارتباط معنادار آماری داشته است و میانگین و انحراف معیار نمره در بیماران فوت‌شده بیشتر بوده است (0/000 =P ). همچنین این ابزار می‌تواند به میزان 72/4 درصد میزان مرگ و میر بیماران کووید-19 را پیش‌بینی کند. نمره آپاچی2 با سن و وضعیت اینتوبیشن بیمار مرتبط بود (0/001 =P )، اما با جنس و مدت بستری ارتباط معنادار آماری نداشته است (0/905 =P ). میزان نمره PSI با مرگ و میر بیماران ارتباط معنادار آماری داشته است و میانگین و انحراف معیار نمره در بیماران فوت‌شده بیشتر بوده است (0/001 =P ) همچنین میزان نمره PSI با مدت بستری بیماران ارتباط معنادار آماری نداشته است (0/581 = P). این ابزار می‌تواند به میزان 66 درصد میزان مرگ و میر بیماران کووید-19 را پیش‌بینی کند. همچنین بر اساس نتایج ارزیابی اخیر مدت بستری با سن، جنس، وضعیت اینتوبیشن بیمار و با سطح هوشیاری اولیه مرتبط نبود. نتیجه‌گیری: استفاده از ابزار آپاچی2 در پذیرش بیماران مبتلا شده به کووید-19 در ICU نسبت به سایر ابزارها پیش‌بینی دقیق‌تری دارد و می‌تواند تا حد زیادی به استفاده بهینه از تخت‌های ICU کمک کند.</div> Background & aim: In Intensive Care Units (ICUs), several systems are used to predict patient mortality. In this study, the relationship between the Apache 2 and PSI scores with the length of stay and mortality rate of Covid-19 patients admitted to the ICU was investigated. Methods: This cross-sectional analytical study was conducted on patients admitted to the ICU. In order to collect information, the PSI and Apache 2 tools were used. After obtaining a referral from the university's research vice chancellor, the hospital archives unit was referred. Then, the data required for the Apache 2 tool, the PSI tool, and demographic characteristics related to the first 24 hours of admission to the ICU were collected from the hospital's files and HIS. Information related to the time of death, length of stay, and patient transfer were also recorded. Results: Of the 486 investigated patients, the mean and standard deviation of age was 62.76 ± 18.1. In addition, 46.7% of patients were female (227 patients) and 53.3% (259 patients) were male. The mean duration of hospitalization in the ICU was 7.15 ± 4.6 days (minimum 1, maximum 27 days). Furthermore, 21% (102 patients) were intubated at the time of examination. Finally, 51.9% (252 patients) were discharged from the ward and 48.1% (234 patients) died. In a recent evaluation, it was observed that the Apache 2 score had a statistically significant relationship with patient mortality, and the mean and standard deviation of the score were higher in deceased patients (P = 0.000). This tool can also predict the mortality rate of Covid-19 patients by 72.4%. The Apache 2 score was related to the patient's age and intubation status (P = 0.001), but it was not statistically significantly related to gender and length of hospitalization (P = 0.905). The PSI score had a statistically significant relationship with patient mortality, and the mean and standard deviation of the score were higher in deceased patients (P = 0.001). The PSI score also had no statistically significant relationship with length of hospitalization (P = 0.581). This tool can predict the mortality rate of Covid-19 patients by 66%. Also, according to the results of the recent evaluation, length of hospitalization was not related to the patient's age, gender, intubation status, and initial level of consciousness.
