fa صلاحیت بالینی پرستاران در اصول تهویه مکانیکی؛ طراحی و اعتباریابی آزمون بالینی ساختار یافته عینی تخصصي Special پژوهشي Research <span style="font-size:12pt"><span style="text-autospace:none"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><span class="StyleLatinHelveticaComplex2LotusLatin12ptComple" new="" roman="" style="font-family:" times=""><span style="color:black"><b><span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"><span style="color:black">زمینه و هدف:</span></span></span></b></span></span> <span lang="AR-SA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"><span style="color:black">ارزشیابی بالینی یکی از اجزاء اصلی و مهم در بحث آموزش بالینی در حوزه پرستاری است. بر اساس هرم سطوح یادگیری میلر، آزمون بالینی ساختار یافته عینی یکی از روش‌های ارزشیابی کاربردی برای ارزشیابی مهارت‌های بالینی محسوب می‌شود. یکی از مهارت‌های بالینی پرستاران بخصوص در بخش مراقبت‌های ویژه تهویه مکانیکی است. این مطالعه با هدف طراحی آزمون بالینی ساختاریافته عینی در ارزشیابی صلاحیت بالینی پرستاران در تهویه مکانیکی انجام شد.</span></span></span><span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"><span style="color:black"></span></span></span></span></span></span></span></span><br> <span style="font-size:12pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><span class="StyleLatinHelveticaComplex2LotusLatin12ptComple" new="" roman="" style="font-family:" times=""><span style="color:black"><b><span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"><span style="color:black">روش‌ها:</span></span></span></b></span></span> <span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"><span style="color:black">این مطالعه روش‌شناسی در شهر تهران، ایران در سال 1401 انجام شد. متناسب با محتوای آموزشی اصول تهویه مکانیکی پایه و هدف مطالعه، شش ایستگاه تعیین و به تناسب برای هر ایستگاه چک لیست تخصصی طراحی شد. بررسی روایی محتوای چک ‌لیست‌های طراحی شده با استفاده از نظر صاحب‌ نظران به دو صورت کیفی و کمّی و پایایی بین ارزیابان انجام شد.</span></span></span><span dir="LTR" style="font-size:11.0pt"><span style="color:black"></span></span></span></span></span></span><br> <span style="font-size:12pt"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><span class="StyleLatinHelveticaComplex2LotusLatin12ptComple" new="" roman="" style="font-family:" times=""><span style="color:black"><b><span lang="AR-SA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"><span style="color:black">یافته‌ها:</span></span></span></b></span></span> <span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:">در نهایت تعداد شش ایستگاه با 31 گویه با عناوین؛ اتصالات و آماده‌سازی اولیه دستگاه تهویه مکانیکی، تنظیمات اولیه و اصولی دستگاه تهویه مکانیکی، بررسی و مداخله در وضعیت اکسیژناسیون، بررسی و مداخله در وضعیت ونتیلاسیون، مدیریت چالش‌های بالینی و مدیریت آلارم‌ها نهایی شد. نحوه نمره‌دهی هر آیتم 5 -1 بوده و جمع نمرات ایستگاه‌ها عنوان‌کننده میزان صلاحیت بالینی پرستار در تهویه مکانیکی خواهد بود.<span style="color:black"></span></span></span></span></span></span></span><br> <span class="StyleLatinHelveticaComplex2LotusLatin12ptComple" new="" roman="" style="font-family:" times=""><span style="color:black"><b><span dir="RTL" lang="AR-SA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"><span style="color:black">نتیجه‌گیری:</span></span></span></b></span></span> <span dir="RTL" lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"><span style="color:black">با توجه به روایی و پایایی مناسب، استفاده از آزمون ساختاریافته بالینی عینی طراحی شده برای ارزشیابی صلاحیت بالینی پرستاران در تهویه مکانیکی پایه پیشنهاد می‌شود.</span></span></span> <pre style="text-align:justify"> <span style="font-size:12pt"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b><span lang="EN" style="font-size:10.0pt">Background & aim:</span></b><span lang="EN" style="font-size:10.0pt"> Clinical evaluation is one of the main components in the discussion of clinical education in the field of nursing. Based on Miller&#39;s Pyramid in Education, objective structured clinical examination becomes one of the applied assessment methods to evaluate clinical skills. One of the clinical skills of nurses, especially in the Intensive Care Unit (ICU), is mechanical ventilation. This study was conducted with the aim of designing an objective structured clinical examination to evaluate the competence of nurses in mechanical ventilation.</span></span></span></span> <span style="font-size:12pt"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b><span lang="EN" style="font-size:10.0pt">Methods:</span></b><span lang="EN" style="font-size:10.0pt"> This descriptive study was conducted in 1401 in Tehran, Iran. According to the educational content of basic mechanical ventilation principles and the purpose of the study, six stations were determined and a specialized checklist was designed accordingly for each station. Checking the validity of the content of the designed checklists was done using the opinions of experts in both qualitative and quantitative ways.</span></span></span></span> <span style="font-size:12pt"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b><span lang="EN" style="font-size:10.0pt">Results:</span></b><span lang="EN" style="font-size:10.0pt"> Finally, the number of six stations with 31 items with titles; Tubing connection and initial preparation of the ventilator, initial and basic settings of the ventilator, check and intervention in the oxygenation status, check and intervention in the ventilation status, management of clinical challenges and management of alarms were finalized.
