fa ارتباط مشخصات بالینی و فردی بیماران بستری در بخش‌های مراقبت ویژه با ابتلا به ترومبوز وریدهای عمقی اندام تحتانی: یک مطالعه مورد- شاهدی Correlation between Clinical and Personal Characteristics of Patients Admitted to Intensive Care Units with Deep Vein Thrombosis of the Lower Extremities تخصصي Special پژوهشي Research زمینه و هدف: ابتلا به ترومبوز وریدهای عمقی یکی از شایع‌ترین مشکلاتی است که در بخش‌های مراقبت ویژه مشاهده می‌شود. مطالعه حاضر با هدف بررسی ارتباط مشخصات بالینی و فردی با ابتلا به ترومبوز وریدهای عمقی اندام تحتانی در بیماران بستری در بخش‌های مراقبت ویژه انجام شد. روش‌ها: پژوهش حاضر یک مطالعه مورد- شاهدی که بر روی 504 بیمار بستری در بخش‌های مراقبت ویژه بیمارستان الزهرا (س) اصفهان طی سال‌های 1398-1393 انجام شده است. نمونه‌ها در گروه مورد 89 به صورت آسان و در گروه شاهد 415 به صورت تصادفی منظم وارد مطالعه شدند. داده‌ها با استفاده از پرسشنامه دو قسمتی مشخصات فردی و بالینی جمع آوری شد، سپس داده‌ها با استفاده از آزمون‌های آماری تی دو نمونه‌ای مستقل، من ویتنی یو، کای اسکور و همچنین رگرسیون لجستیک تحلیل شد. یافته‌ها: میانگین سنی شرکت کنندگان در حدود 53 سال بود، اگرچه میانگین سنی در گروه مورد بیشتر از گروه شاهد بود (0/004=p) اما پس از کنترل تأثیر جنس و ابتلا به بیماری‌های زمینه‌ای، تأثیری بر ابتلا به ترومبوز نداشت (075/0=p) شانس ابتلا به ترومبوز در مردان 1/7 برابر زنان بود (0/029=p) شاخص pao2/fio2 در مبتلایان به ترومبوز (0/02±0/61) به طور معناداری کمتر از گروه غیر مبتلایان (2/1±5/5) بود (0/0001>p) پس از کنترل اثر سن و جنس، شانس رخداد ترمبوز در افراد مبتلا به پرفشاری خون، دیابت، آمبولی ریه و سابقه قبلی ابتلا به ترومبوز به ترتیب (0/0001>p) 2/6، (0/0001>p) 3/3، (0/006=p) 2/1، (0/0001>p) 8/1 و (0/0001>p) 36/3 بود. بی‌تحرکی با شانس(0/0001=p) 29/9 مهمترین عامل خطر در آی سی یو برای ابتلا به ترومبوز بود. نتیجه‌گیری: نتایج این مطالعه نشان می‌دهد که داشتن برخی ویژگی‌های خاص و ابتلا به برخی بیماری‌های زمینه‌ای که بواسطه آنها بیماران در بخش‌های ویژه بستری می‌شوند، شانس ابتلا به ترومبوز در وریدهای عمقی را افزایش می‌دهد. بنابراین پرستاران می‌توانند با آگاهی در برنامه‌ریزی‌های پیشگیرانه آنها را مورد توجه بیشتر قرار داده و از بروز آنها بکاهند.   Background and Aim: Deep Vein Thrombosis (DVT) is one of the most common problems in Intensive Care Units (ICU). The aim of this study was to investigate the relationship between clinical and demographic characteristics with lower extremity DVT in patients admitted to ICU. Methods: The present study is a retrospective case-control study that was performed on 504 patients admitted to the ICUs of Al-Zahra Hospital in Isfahan during 2014-2019. Samples were easily included in the case group (89) and in the control group (415), randomly. Data were collected using a two-part questionnaire of demographic and clinical characteristics, then descriptive and analytical statistical tests such as two-sample independent t-test and if necessary Mann-Whitney were used and were eventually analyzed with SPSS version 21. Results: The results showed that the mean age of participants was about 53 years, although the mean age in the case group was higher than the control group (p=0.004), but after controlling sex and underlying diseases, had no effect on thrombosis (0.075). The chance of developing thrombosis in men was 1.7 times higher than women (p=0.029). The pao2 / fio2 index in patients with thrombosis (0.61±0.02) was significantly lower than the control group (5.5±2.1) (p<0.0001). After controlling for the effect of age and sex, the chance of thrombosis in patients with hypertension, diabetes, pulmonary embolism and previous history of thrombosis (p<0.0001), 2.6 (p<0.0001), respectively, 3.3 (p=0.006), 1/2 (p<0.0001) 8.1 (p<0.0001) was 36.3. Inactivity with a chance of 29.9 (p=0.0001) was the most important risk factor in ICU for thrombosis. Conclusion: The results of this study show that having certain characteristics and having some underlying diseases due to which patients are admitted to special wards, increases the chance of developing DVT.
