بررسی میزان خشونت بیمار و خانواده آنها علیه پرستاران اورژانس

اهداف : خشونت بیمار و ملاقات کننده او اغلب جزء خطرناکترین مخاطرات شغلی است و برای کارکنان تبعاتی به دنبال دارد که در نهایت مستقیم یا غیرمستقیم، بر کیفیت مراقبت بیمار و رضایتمندی بیماران تأثیر میگذارد. مطالعه حاضر با هدف تعیین میزان خشونت بیمار و خانواده آنها علیه پرستاران انجام شد. روشها: این مطالعه توصیفی ـ مقطعی بر روی 68 نفر پرسنل پرستاری شاغل در بخش اورژانس مرکزی بیمارستان امام رضا (ع) در مشهد به روش سرشماری، در سال 1391، انجام شد. ابزار گردآوری دادهها، پرسشنامه استانداردی است که توسط همکاری سازمان جهانی کار، سازمان جهانی بهداشت، انجمن بینالمللی پرستاران و انجمن بینالمللی خدمات عمومی طراحی شده است که پس از تعدیل بر اساس شرایط محیطی و اجتماعی، مورد استفاده قرار گرفت. دادهها با نرمافزار آماری SPSS 17 و با استفاده از آمار توصیفی و آزمون همبستگی پیرسون و اسپیرمن مورد تجزیه و تحلیل قرار گرفت. یافته ها: همه پرستاران در یک سال گذشته حداقل یک بار با خشونت کلامی مواجهه داشتند (7/14 درصد یک بار، 2/16 درصد همیشه و 1/69 درصد گاهیاوقات) و 1/22 درصد از آنها نیز خشونت فیزیکی را در یک سال گذشته تجربه کرده بودند. ﺑﻴﺸﺘﺮﻳﻦﻋﺎﻣـﻞ ﺧﺸﻮﻧﺖ ﻋﻠﻴﻪ ﭘﺮﺳﺘﺎران ﻫﻤﺮاﻫﺎن ﺑﻴﻤﺎر ﺑﻮدند؛ بیشتر پرستاران در مقابل آنان هیچ اقدامی انجام نداده بودند. بیش از نیمی از پرﺳﺘﺎﺭﺍﻥ اعلام کردند که ﺑﻪ ﺩﻟﻴﻞ ﺍﺣﺴﺎﺱ ﺑﻲفاﻳﺪﻩ ﺑﻮﺩﻥ ﮔﺰﺍﺭﺵﺩﻫﻲ ﻳﺎ ﺻﺤﺒﺖ ﮐﺮﺩﻥ ﺩﺭ ﺍﺭﺗﺒﺎﻁ با واقعه، ﺁﻥ ﺭﺍﮔﺰﺍﺭﺵ ﻧﻤﻲﺩﻫﻨﺪ ﻭ ذکر شده که حتی در صورت گزارشدهی هیچ اقدام خاصی توسط مافوق برای شناسایی علت خشونتها صورت نپذیرفته است. به علاوه پیگیری بیشتر خشونتهای گزارش شده غالبا رضایتبخش نبوده است. نتیجهگیری: با وجود شیوع بالای خشونت علیه پرستاران در بخشهای اورژانس، عدم وجود سازوکارهای گزارش و پیگیری خشونتهای اتفاق افتاده و اجتناب پرستاران از گزارش خشونتها، زوایای پنهان این پدیده بیش از بخش آشکار آن است. از آنجا که اغلب شاغلین بخش اورژانس خشونت را غیر قابل پیشگیری گزارش نمودهاند، لازم است باتوسعه برنامههای آموزشی با هدف توانمندسازی پرستاران در مدیریت وقایع خشونتآمیز از اثرات مخرب این پدیده بر کیفیت مراقبتهای اورژانس پیشگیری نمود.
Aims: Patients and their families’ violence is mostly among the most dangerous occupational hazards, which has some consequences for the medical staff and influences quality of patients’ care and satisfaction directly or indirectly. The present study is done with the aim of determining the level of patients and their families’ violence against nurses. Methods: This cross - sectional study was performed through census method on 68 nursing staff working in Central Emergency of Imam Reza hospital in Mashhad in 2012. Data collection tools included a standard questionnaire designed by the collaboration of International Labor Organization, World Health Organization, International Society of Nurses and International Association of Public Service the questionnaire has been used after its adjustment with the social and environmental criteria. Data analysis was done through SPSS17 software and by using descriptive statistics and Pearson and Spearman correlation test. Results : All the nurses were exposed to verbal violence at least once during the last year (14.7 percent once, 16.2 percent always and 69.1 percent sometimes) and 22.1 percent of them had experienced physical violence during the last year. The most common cause of violence against nurses was patients' relatives and most of the nurses have not taken any action against them. More than half of the nurses stated that they do not report the incident since they think it’s useless to report or talk about it they also stated that there was no specific action taken by the superior for identifying the cause of violence in the case of their reporting. In addition tracking the reported violence has most often been unsatisfactory. Conclusions: Despite high prevalence of violence against nurses of emergency departments, lack of reporting mechanisms and tracking violence and nurses’ avoidance of reporting violence, hidden aspects of this phenomenon are more than the obvious ones. Since most of the people working in emergency departments have reported that violence can’t be prevented, it is necessary to prevent destructive effects of this phenomenon influencing the quality of emergency cares by developing educational programs aimed at empowering nurses in managing violent incidents.
