مقایسه عملکرد بخشهای مراقبت ویژه براساس میزان مرگ و میر و استانداردهای ساختاری در بیمارستانهای آموزشی

اهداف: رسالت بخشها ی مراقبت ویژه بیمارستانها کاهش میران مرگ و میر و ارتقاء سلامت جامعه میباشد، لذا این مطالعه به منظور " بررسی و مقایسه عملکرد بخشهای مراقبت ویژه با تعیین میزان مرگ و میر و ارتباط آن با وضعیت ساختاری بخشهای مذکور " انجام شد. مواد و روشها : این مطالعه توصیفی مقطعی در یک دوره شش ماهه در سال 1390 در بخش های مراقبت ویژه بیمارستانهای آموزشی شهر همدان انجام شد. مطالعه به روش نمونه گیری در دسترس دربخش مراقبت ویژه بیمارستان بعثت و دو بخش مراقبت ویژه بیمارستان شهید بهشتی بر روی تعداد 200 بیمارانجام گردید. ابزارهای گردآوری داده ها شامل چک لیست ساپس سه ( SAPS3 ) که روایی و پایایی آن در مطالعات مختلف تائید گردیده و فهرست وارسی استانداردهای ساختاری محقق ساخته در سه حیطه ( نیروی انسانی، محیط فیزیکی و تجهیزات پزشکی) بودند. نسبت میزان مرگ ومیر واقعی به مرگ ومیر مورد انتظار توسط مقیاس SAPS3 تعیین وارتباط آن با نمره استاندارد ساختاری سنجیده شد. از آزمونهای آماری کروسکالوالیس، من ویتنی، مجذور کای و ضریب همبستگی اسپیرمن و نرم افزار 17 SPSS جهت تجزیه و تحلیل داده ها استفاده شد. یافته ها: تمام بخش های مورد مطالعه بر اساس سیستم نمره دهی ساپس سه، نسبت مرگ و میر واقعی به مرگ و میر مورد انتظار SMR) ) بیشتر از یک داشتندکه به معنای عملکرد ضعیف است، بطوریکه SMR بخش بعثت 29/2 ، بخش قائم 38/1 و بخش میلاد 56/1 بود. نمره وضعیت ساختاری بخشهای مورد مطالعه نیز کمتر از 50 % استاندارد بود بطوریکه بخش بعثت 48 %، بخش قائم 45 % و بخش میلاد 41 % نمره استانداردهای ساختاری را دارا بودند. بین امتیاز استانداردهای ساختاری بخشهای مورد مطالعه و نمره عملکردی( SMR ) آنها ارتباط معنی داری مشاهده نشد ( 66/0 P: ) نتیجه گیری: یافته های بدست امده حاکی از انست که نمره عملکردی هرسه بخش ضعیف وبخشهای مذکور از نظر استانداردهای ساختاری در حد کمتر از متوسط بوده اند. لذا لازمست تلاش در جهت بهبود استانداردهای ساختاری و کاهش نسبت مرگ و میر واقعی به مرگ و میر مورد انتظار، مورد توجه مدیران، پزشکان، پرستاران و سایر کارکنان بخش های مراقبت ویژه قرار گیرد .
Aims: The aim of intensive care units of the hospitals is decreasing mortality rate and improving health of the society. This study was done with the aim of “studying and comparing the performance of intensive care units by determining mortality rate and finding its relationship with structural situation of these units”. Methods: This cross-sectional study was done during six months in 2011 in the intensive care units of educational hospitals in Hamadan. The study was done on 200 patients through convenient sampling in the intensive unit of Besat hospital and in two intensive care units of Shahid Beheshti hospital. Data collection tools included SAPS3 checklist that its validity and reliability were confirmed in different studies and a researcher-made structural standards checklist in three areas (human force, physical environment and medical equipment). The ratio of the actual death to the expected death (SMR: standardized mortality rate) was determined by SAPS3 and its relationship with the structural standard score was measured. For data analysis, Kruskal-Wallis, Mann-Whitney, chi-square and Spearman correlation coefficient and SPSS17 software were used. Results: Based on the SAPS3 scoring system, the ration of the actual death to the expected death (SMR) was more than one in all the units of the study which means weak performance in a way that SMR of Besat, Ghaem and Milad units were respectively 2.29, 1.38 and 1.56. Structural situation score of the units of the study were also less than 50% of the standard in a way that Besat, Ghaem and Milad units had respectively 48%, 45% and 41% of the structural standards scores. There was no significant relationship between structural standards scores of the units of the study and their SMR performance score (p=0.66). Conclusions: Performance score of all the three units was weak and regarding structural standards the mentioned units were less than moderate. It is necessary to try to improve structural standards and to decrease ratio of the actual death to the amount of death expected by managers, physicians, nurses and other staff of the intensive care units.
