fa پاسخ پرستاران بخش های ویژه به تنیدگی اخلاقی: یک مطالعه ی کیفی عمومى General پژوهشي Research چکیده اهداف: تنیدگی اخلاقی سبب بروز مجموعه­ای از پاسخ­ها در پرستاران می­شود. با شناخت این پاسخ­ها می­توان راهبردهایی برای رویارویی بهتر پرستاران با آن­ها بکار برد. این مطالعه به منظور «تبیین پاسخ پرستاران بخش مراقبت ویژه به تنیدگی اخلاقی» انجام شد. روش­ها: این مطالعه کیفی با رویکرد تحلیل محتوا در نیمه دوم سال 1389 انجام شد. داده­ها با استفاده از مصاحبه­های بدون ساختار و عمیق با سیزده مصاحبه انفرادی و دو مصاحبه گروهی با پرستاران بالینی شاغل در بخش­های مراقبت ویژه­ی شهرهای مختلف ایران گردآوری شد. نمونه­گیری به صورت هدفمند و تجزیه و تحلیل داده ­ ها با تجزیه و تحلیل محتوای کیفی انجام شد. یافته­ها: طی فرآیند تحلیل محتوا سه مضمون، پنج زیرمضمون و 28 طبقه حاصل شد . این مضامین شامل " واکنش­های روان­تنی "، " واکنش­های معنوی " و "روش­های مقابله"ی پرستاران در پاسخ به تنیدگی اخلاقی در بخش مراقبت ویژه بود. نتیجه­گیری: یافته­های این پژوهش نشان داد که پرستاران بخش ویژه در رویارویی با تنیدگی اخلاقی از پاسخ­های گوناگونی نظیر واکنش­ها و روش­های مختلف مقابله­ای استفاده می­نمایند که برخی از این پاسخ­ها مثبت و برخی منفی هستند. مسئولین پرستاری می­توانند به پرستاران برای کاهش تنیدگی­ها، استفاده از روش­های مثبت مقابله­ای و پیشگیری از پیامدهای مضر آن کمک نمایند. Abstract Purposes of the study: Moral distress elicits some responses from nurses. Identifying these responses can lead into developing effective strategies which assist nurses with facing them. The present study tries to elicit responses of ICU nurses to moral distress. Methodology: This is a qualitative research with an analytical approach. The required data was gathered through deep and unstructural interviews (retrospective interviews). 13 individual interviews and two group interviews were conducted with clinical nurses working in ICUs of different cities in Iran. Samples were taken purposefully and data analysis was carried out by analyzing the qualitative content. Findings: A nalyzing the content, 3 central themes, 5 subthemes, and 28 categories were obtained. The main themes included "psychosomatic reactions", "spiritual reactions", and "coping methods” of nurses in response to moral distress in ICUs. Conclusion: the obtained results of this study asserted that facing moral distress, ICU nurses produce various reactions and adopt alternative coping methods, some of which are positive and some are not. Applying positive coping methods and preventing harmful outcomes, nursing administrators can help ICU nurses to reduce moral distress.
