abstract ={Aims: Many patients hospitalized in CCU experience reduced quality of sleep. One of the methods for treating sleep disorders is using complementary medicine such as acupressure. The purpose of this study is “investigating the effect of acupressure on the quality of sleep in patients with Acute Coronary Syndrome (ACS) in CCU”. Methods: This is a two-group double blind clinical trial study which was conducted on 60 patients with Acute Coronary Syndrome (ACS) in Mazandaran Heart Center in 2013. Samples were selected from statistical available population and were randomly assigned in two groups. The group of acupressure received bilateral acupoint massage from the second night of hospitalization for two minutes during three nights, in every point it was totally 18 minutes. Patients in the control group received massage on these points with the same technique and time with 1-1.5 cm distance from the main points. The quality of patients’ sleep was completed with SMHSQ hospital sleep questionnaire. Findings were analyzed by SPSS18 software and χ2 , independent t-test, RM-ANOVA statistical tests. Results: There was significant difference between sleep quality of the patients in acupressure group and control group which was also statistically significant (p <; 0.05). Conclusions: This study showed that acupressure can have therapeutic effect in improving quality of sleep in ACS patients. Therefore this method can be taught to nurses who have an important role in the identification and elimination of patients’ sleep disorder and also it should be taught to the patients themselves and their companions .},

abstract ={Aims: Pain due to the removal of the chest tube is one of the important complications after open heart surgery. In the case of inadequate pain management, sympathetic system is stimulated and can lead to irreversible complications. Studies showed the effect of reflexology massage on pain relief in other cases so this study had been done with the aim of “determining the effect of foot reflexology on the pain of the patients under open heart surgery during chest tube removal”. Methods: This randomized clinical study with control group was done in the hospitals covered by Baqiyatallah Medical Sciences University in 2013. Ninety samples were divided into three experimental, control and placebo-treated groups based on randomized allocation. Pain level was measured through Numerical Rating Scale (NRS) in all the three groups before intervention. In the experimental group center of the anterior one-third and in the placebo-treated group, Posterior one-third of the left foot was being massaged for ten minutes before chest tube removal. There was no measurement in the control group. After removal of the chest tube, the level of the pain was measured and documented immediately. Data were analyzed by SPSS18 software by the help of descriptive and inferential statistic s . Results: Difference in the mean of the quantitative variables including age, height, weight and level of the body and demographic qualitative variables of the patients including education status, occupation and marital status was not significant (p>0.05). Expected increasing of the pain due to the chest pain tube removal was not significant in the experimental group (p=0.08), while placebo-treated and control groups had significant increase of the pain (p=0.001 and p=0.000 respectively). Conclusions: Foot reflexology massage is a useful nursing intervention in chest tube removal after open heart surgery. This intervention improves care level of the patients in the important recovery phase after open heart surgery with the least cost and without any complications.},

abstract ={Aims: Prematurely birth of the infant and his/her hospitalization in NICU is something really unexpected for the parents. In most of the cases fathers are the first visitors of the infant in NICU and they are counted as the key person in supporting mother and infant. Lack of supporting father and shortage of his awareness increase his stress in this crisis and have some inappropriate short-term and long-term outcomes. The aim of this study was “assessing the effect of a designed program about the intervention of family support on the stress of the fathers of premature infants hospitalized in NICU”. Methods: In a two-group semi-experimental study which was done in 2013, fathers of fifty premature infants hospitalized in NICU of Shahid Beheshti hospital in Isfahan were chosen through convenient sampling and were divided into two control and experimental group. Family supporting program was a two-phase program. The first phase included two informational and one observational sections and the second group included supporting section. Parental stress measurement questionnaire in both of the groups was completed by the infant’s father in two before and after time of intervention. For analyzing information SPSS18 software and descriptive and inferential tests were used. Results: In the experimental group there was significant difference in the mean of scores before and after intervention in two subscales of “appearance and behavior of the infant and special treatments” and “the relationship of the parents with the infant and acceptance of parental roles” and the mean of total score, while this difference was not significant in control group in none of subscales of parental stress and total score. Before intervention, there was no significant difference between the two groups in terms of the mean score of parental stress in three subscales of parental stress and total score but after intervention in experimental group, the difference of the mean of the parental stress scores in two subscales of infant’s appearance and behavior and special treatments and the relationship of the parents with the infant and acceptance of parental roles and total score was significantly less than control group. Conclusions: Emotional-informational support program could decrease stress level of the fathers of premature infants hospitalized in NICU, so appropriate effort and policies can be valuable in paying more attention to the father and his partnership.},

abstract ={Aims: The aim of intensive care units of the hospitals is decreasing mortality rate and improving health of the society. This study was done with the aim of “studying and comparing the performance of intensive care units by determining mortality rate and finding its relationship with structural situation of these units”. Methods: This cross-sectional study was done during six months in 2011 in the intensive care units of educational hospitals in Hamadan. The study was done on 200 patients through convenient sampling in the intensive unit of Besat hospital and in two intensive care units of Shahid Beheshti hospital. Data collection tools included SAPS3 checklist that its validity and reliability were confirmed in different studies and a researcher-made structural standards checklist in three areas (human force, physical environment and medical equipment). The ratio of the actual death to the expected death (SMR: standardized mortality rate) was determined by SAPS3 and its relationship with the structural standard score was measured. For data analysis, Kruskal-Wallis, Mann-Whitney, chi-square and Spearman correlation coefficient and SPSS17 software were used. Results: Based on the SAPS3 scoring system, the ration of the actual death to the expected death (SMR) was more than one in all the units of the study which means weak performance in a way that SMR of Besat, Ghaem and Milad units were respectively 2.29, 1.38 and 1.56. Structural situation score of the units of the study were also less than 50% of the standard in a way that Besat, Ghaem and Milad units had respectively 48%, 45% and 41% of the structural standards scores. There was no significant relationship between structural standards scores of the units of the study and their SMR performance score (p=0.66). Conclusions: Performance score of all the three units was weak and regarding structural standards the mentioned units were less than moderate. It is necessary to try to improve structural standards and to decrease ratio of the actual death to the amount of death expected by managers, physicians, nurses and other staff of the intensive care units.},

title = {Prevalence of Functional Iron Deficiency (FID) Anemia in Patients Undergoing Hemodialysis},
abstract ={Aims : Anemia is one of the main problems in hemodialysis patients, which is caused by inadequate production of erythropoietin. Functional iron deficiency (FID) anemia is a kind of anemia, which lack of functional iron therein leads to resistance and inappropriate response to erythropoietin in hemodialysis patients. Since early diagnosis of this anemia before erythropoietin usage is important, this study was conducted “to determine p revalence of functional iron deficiency in hemodialysis patients”. Methods: In a cross–sectional design studied 184 patients with chronic kidney disease referred to hemodialysis units in Baqiyatallah and Chamran hospitals in Tehran. In order to data collecting were measured serum Hb, Hct and Ferritin levels plus to completing the demographic questionnaire. The χ2-test, T-test and ANOVA were used in this study. The data were analyzed via the SPSS18 software (version). The p value of 0.05 was considered as significance level. Results: The mean of serum levels of Hb and Hct was respectively 10.98±1.7 g/dl and 34.1±5.2%. Anemia was observed in 37%, hyperferritinemia in 80.4% and functional iron deficiency anemia in 41.1% of patients. Conclusions: Functional iron deficiency was observed in about half of hemodialysis patients and could cause resistance and inappropriate response to Erythropoetin in them. Therefore nurse awareness and his/her duly action in determining the status of iron stores prior to the administration of erythropoietin prevents from prescribing an expensive drug and imposing unnecessary costs to the patient and the health care system.},

abstract ={Aims: Acute Coronary Syndrome (ACS) is a common cardiovascular disease. In patients with this disease, symptom management is performed both pharmacologically and non-pharmacologically. One of the non-pharmacologic symptom management interventions is chest local heat therapy. The aim of this study was “to determine the effect of chest local heat therapy on physiologic parameters in patients with ACS”. Methods: We conducted this randomized double-blind placebo-controlled clinical trial in 2013—during six months in Cardiac Care Unit of 22 Bahman hospital in Gonabad, Iran. In this study, we randomly allocated 66 patients with ACS to two placebo and experimental groups. Patients in the experimental and control groups received chest local heat therapy by using a hot pack warmed to 50°C and 37°C respectively. Local heat therapy was administered for four 23-minute sessions in the anterior part of the chest. The levels of patients’ heart rate, respiratory rate, arterial blood pressure and blood oxygen saturation were measured both before and after the intervention. Data collection tools consisted of a demographic questionnaire and a data sheet for recording physiologic parameters. Data were analyzed by using SPSS 16 software and descriptive and inferential statistical tests. Results: There was no significant difference in the two groups before and after the intervention in terms of systolic and diastolic blood pressure as well as heart rate. However, in compare with before the intervention, systolic and diastolic blood pressure in the experimental group were decreased significantly (p<; 0.05). Moreover, before the intervention, there was no significant difference in the two groups in terms of respiratory rate and oxygen saturation. However, after the intervention, respiratory rate in the experimental group in compare with before the intervention and placebo group were decreased significantly and oxygen saturation was increased significantly (p<; 0.05). Conclusions: The study findings confirmed the effectiveness of local heat therapy in improving physiologic parameters in patients with ACS.},

title = {Comparing the effect of using normal saline, N-acetyl cysteine and not using them in endotracheal tube suction on physiologic parameters and the amount of secretions in intubated patients under mechanical ventilation},
abstract ={Aims: Today a large number of patients hospitalized in Intensive Care Unit need mechanical ventilation via an artificial air way. Since these patients have endotracheal tube, endotracheal suctioning is necessary for increasing oxygenation, cleaning the airway and keeping it open . But this method consists of various side effects such as increased blood pressure and heart rate and respiratory rate, cyanosis, dizziness and increased intracranial pressure and hypoxia . This study aimed at comparing the effect of using normal saline serum, N-acetyl cysteine and not using them in endotracheal suctioning on physiologic parameters and the amount of secretions in intubated patients under mechanical ventilation. Material & Methods: It was a controlled clinical trial study with random allocation that was performed on 54 intubated patients hospitalized in ICU of health educational centers of Babol Medical Sciences University in 2014. Endotracheal tube suctioning was performed for each patient once without saline, the second time with 3 ml of normal saline and the third time with 2 ml of N-acetyl cysteine. Blood pressure, Heart rate, Respiratory rate, Arterial oxygen saturation were measured and recorded before each stage of suctioning and two and five minutes after that. Amount of secretions was measured after each method of suctioning. Data were analyzed by SPSS16 statistical software, variance analysis with repeated measures, ANOVA and paired t statistical tests. Results: Blood pressure, Heart rate and Respiratory rate were increased after suction in all the three groups, but these changes were higher in the group that received normal saline. Average decrease of Arterial oxygen saturation was more in the method of receiving normal saline (p<; 0.05). The amount of secretion was more in the method of receiving N-acetyl cysteine (p=0.004). Conclusions: Endotracheal tube suctioning by using normal saline had more adverse effects on post- suction physiological parameters in compare with the other two methods . Therefore, it is recommended to not use normal saline in endotracheal tube suctioning to the possible extent , and if removing secretions is required, endotracheal N_ acetyl cysteine should be used according to the patients condition .},

abstract ={Aims: Considering the increasing rate of hemodialysis patients , different results of previous studies , human’s constant interaction with the environmental changes and living in today’s modern world, people are exposed to a barrage of constant changes and stressors . These changes in chronic renal failure patients comparing to healthy individuals are more and different . The type of coping strategies used by the people depend on several factors including personal experience , social support systems , personal beliefs , available resources and genetic background .The study aimed to “identifying the type of experienced stressors and coping strategies used by hemodialysis patients”. Mothods : In this descriptive correlational study, Hemodialysis Stressors Scale (HSS) and Jalowiec Coping Scale (JCS) were used to investigate stressors and coping strategies of 70 hemodialysis patients hospitalized in hospitals supervised by Medical Sciences University of Jiroft in 2013. Sampling was done through census sampling method. Data analysis was done by SPSS 20 software , the methods of descriptive and inferential statistics of chi-square type and Fishers exact test with 95 % Confidence Coefficient. Results: The majority of samples ( 48/6%) were from 41 to 60 years old and 60 % of them were male . 11/4 percent of them experienced physiological stressor and 88/6 percent experienced psychosocial stressors . Therefore psychosocial stressors were reported more bothering than physiologic stressors and patients experienced them more. Optimistic coping mechanism was the most common coping strategies used by the patients. The most frequent experienced physiological stressors included fatigue, limitations of fluids, limitations of food and arterial & venous stick and the most frequent psychosocial stressors included limits on the time & place for vacation, boredom, sleep disturbances and interference with job. There was statistically significant relationship between the percentage of experienced stressors in hemodialysis patients and their age (p=0/005). Conclusions: Since psychosocial stressors were reported more bothering than physiologic stressors by the participants, decreasing or balancing them seems so essential. Being familiar with the experienced stressors and the amount of usage and effectiveness of the used coping strategies can help to provide appropriate programs to facilitate adaptation and acquiring necessary skills for patients and their families.},

Written and verbal educational materials about hemodialysis and dietary adherence were provided to patients in the treatment group. Then, the four-step continuous care plan was implemented. The steps included orientation, sensitization, control, and evaluation. Study participants were invited to fill the Dietary Adherence Questionnaire at four time points including before the intervention (T1) as well as one, two, and three months afterward (T2–T4). The Chi-square and the repeated measures analysis of variance test were performed by using the SPSS v. 18.0. Results : Most participants had poor dietary adherence. The Continuous Care Model significantly improved their dietary adherence scores—from 148.95±6.04 (T1) to 156.25±4.85 (T2), 177.08±3.63 (T3), and 184.37±3.38 (T4). There was a significant relationship between Continuous Care Model and dietary adherence (P value = 0.0001). Conclusion s: Educations and counseling services that are provided through the Continuous Care Model can improve hemodialysis patients’ dietary adherence. Using this model for enhancing hemodialysis patients’ dietary adherence and preventing non-adherence-related complications is recommended.},

title = {Evaluation of violence of patients and their families against emergency nurses of Imam Reza Central Hospital in Mashhad},
abstract ={Aims: Patients and their families’ violence is mostly among the most dangerous occupational hazards, which has some consequences for the medical staff and influences quality of patients’ care and satisfaction directly or indirectly. The present study is done with the aim of determining the level of patients and their families’ violence against nurses. Methods: This cross - sectional study was performed through census method on 68 nursing staff working in Central Emergency of Imam Reza hospital in Mashhad in 2012. Data collection tools included a standard questionnaire designed by the collaboration of International Labor Organization, World Health Organization, International Society of Nurses and International Association of Public Service the questionnaire has been used after its adjustment with the social and environmental criteria. Data analysis was done through SPSS17 software and by using descriptive statistics and Pearson and Spearman correlation test. Results : All the nurses were exposed to verbal violence at least once during the last year (14.7 percent once, 16.2 percent always and 69.1 percent sometimes) and 22.1 percent of them had experienced physical violence during the last year. The most common cause of violence against nurses was patients relatives and most of the nurses have not taken any action against them. More than half of the nurses stated that they do not report the incident since they think it’s useless to report or talk about it they also stated that there was no specific action taken by the superior for identifying the cause of violence in the case of their reporting. In addition tracking the reported violence has most often been unsatisfactory. Conclusions: Despite high prevalence of violence against nurses of emergency departments, lack of reporting mechanisms and tracking violence and nurses’ avoidance of reporting violence, hidden aspects of this phenomenon are more than the obvious ones. Since most of the people working in emergency departments have reported that violence can’t be prevented, it is necessary to prevent destructive effects of this phenomenon influencing the quality of emergency cares by developing educational programs aimed at empowering nurses in managing violent incidents.},

title = {The role of attachment styles in the prediction of posttraumatic stress disorder in emergency nurses},
abstract ={Aims: Evidences indicated that attachment styles are important in people’s adjustment with stressful situations. Therefore, this study has been carried out to assess the role of attachment styles in the prediction of Post-Traumatic Stress Disorder (PTSD) in emergency nurses. Methods: It was a descriptive-correlational study. The statistical population of this study was all the emergency nurses of Uroumieh hospitals in 2010. 100 people of the population were selected randomly for this study. All the participants were tested individually by Mississippi PTSD scale and attachment styles questionnaire at their work place. Data analysis was done by multivariate ANOVA tests and multiple regression analysis and SPSS17 software. Results: According to PTSD scale, twenty participants of the study were suffering from PTSD. Comparing their attachment styles with the healthy group (the group without PTSD) it was shown that the group with PTSD took higher scores in terms of fearful and dismissive attachment styles in compare with healthy group (without PTSD). The results of multi-regression analysis showed that 10 percent of the total variance of individuals in PTSD scale was explained by attachment styles. Conclusions: Attachment styles are important in determining susceptibility of hospital emergency nurses to PTSD},

title = {Survey of parents Satisfaction of infants admitted in the NICU},
abstract ={Aims: Despite the importance of patient satisfaction as an indicator of quality, updated information is not available on the satisfaction of parents of infants hospitalized in neonatal intensive care. The aim of this study was “to determine the degree of satisfaction of parents of infants hospitalized in the neonatal intensive care unit (NICU)”. Methods: In this cross-sectional study, parents of 300 infants admitted to the neonatal intensive care unit participated with cluster sampling of twelve elected government hospital in Tehran in 2012. Demographic data and data on parental satisfaction using valid and reliable researcher made questionnaire in three domains of welfare, medical and nursing services was measured with a likert scale ranged from 0-100. Data using descriptive statistics (mean, standard deviation) and inferential statistics (Pearson chi-square) were analyzed in SPSS19 software. Results : The mean satisfaction about the nursing care 63.6±14.5, medical care 54.05±11.9, welfare services 42.03±9.02 and the overall satisfaction 63.04±31.5. A significant relationship was observed between variables hospitals (p=0.0001), age (p=0.014), income (p=0.007), history of hospitalization (p=0.0001) and length of hospitalization (p=0.015) between total satisfaction. Conclusions : The relatively high level of parental satisfaction with the three domains of welfare, medical and nursing care services, can be a desirable indicator of the quality of services provided, and the it is necessary for more parental satisfy , the medical team has more cooperation and assistance together. Doctors and nurses also need to establish closer communication and more intimate with parents. To be considered special amenities and facilities to meet the other family members with newborns.},

title = {Examining the impact of liaison nurse role on patients’ clinical outcomes after intensive care unit discharge: a clinical trial},
abstract ={Aims : The liaison nurse role has been defined in many countries for improving the quality of nursing care delivery to patients who are transferred from intensive care units to general medical-surgical wards. The aim of this study was to examine the impact of ICU liaison nurse role on patients’ clinical outcomes after being discharged from intensive care unit. Methods : In this two-group controlled clinical trial, a convenience sample of 80 patients was drawn in 2014 from selected military hospitals located in Tehran. Patients were randomly allocated to either the control (40 patients) or the experimental groups (40 patients). After being transferred from intensive care unit to general wards, patients in the experimental group received care from a liaison nurse. A data sheet was used for collecting the study data. The validity of the sheet was confirmed by a panel of ten nursing faculties. The Gutmann coefficient of the scale was 0.868 which confirmed the acceptable reliability of the sheet. Study data were analyzed by using the SPSS19 and by conducting the Chi-square and independent-samples t tests. Results : Hemodynamic and laboratory parameters in the experimental group were not significantly different from the control group (p value > 0.05). Conclusions : Conducting further studies for evaluating different aspects of liaison nurse role is recommended},
