abstract ={Abstract Aims: open heart surgery is a stressful experience for the patients and their family. Patients from admission time to discharge experience different level of anxiety and in order to cope with that they use some strategies. This study had been done with the aim of identification of the experiences of open heart surgery patients from admission to discharge. Methods: Samples had been chosen purposefully from 8 women and 10 men who were under open heart surgery in three centers of educational hospitals of Tehran in 2012. Data were collected through unstructured interviews. With qualitative approach of analyzing the content of Graneheim&#59;Lundman 2004had been analyzed. Results: From analyzing data three themes including Stuckat thecrossroadsof deaththreats, seeking a way to stay and forced to compromise with the situation had been emerged. Themes and subthemes indicate anxieties and strategies of the patients in hospital environment. Conclusion: Facing several stressful factors during care period and not meeting physical, psychological and educational needs lead to feeling fear and threat in the patients. Patients’ attempt in these adverse care conditions is self-protection with using different strategies. So real identification of patients’ needs and anxieties is very important in clinical nurses’ care planning.},

title = {Study of education effect on nurses’ knowledge and attitudes about organ donation at the time of brain death}, 
abstract ={Abstract Aims: The frequency of organ transplantation in Iran in compare with European countries and United States is remarkably low and it is because of transplantable organ shortage and because of this thousands of people have been died annually, this is while we encounter with some cases of brain death every day that the organs of these patients with brain death can save life of the patients who are in need of these organs. The aim of this study was “to assess the effect of education on knowledge and attitude of nurses about organ donation”. Methods: This research was a semi-experimental in the form of test-retest study. The group of the study was 120 nurses of ICU and emergency wards of Kerman hospitals who participated in the educational seminar of brain death. Data were collected by a self-designed questionnaire. The questionnaire was completed by the nurses once in the first day of the seminar (before education) and once after presenting related education (after two weeks). Results: Results showed that before education the least knowledge was about three organs of lung, pancreas and marrow that it was 73.3%, 57.5%, and 54.2% respectively, while after education it had been increased 95%, 91.7% and 80% , respectively (P&#60;; 0.001). The average of nurses’ knowledge about process of brain death and donation after education was 9.4 (±0.78) that in compare with before education had 7.5 (± 2.6) significant increase (P&#60;; 0.001). Total average of nurses’ attitude towards process of brain death and organ donation before training course was 65.7 (±13.7) which had been significantly increased to 76.9 (±8.7) two weeks after education (P&#60;; 0.001). Conclusion: The present study showed that more educational programs are necessary for increasing knowledge about brain death and organ donation among health staff especially in ICU and emergency wards that are starter of diagnosis process of brain death patients. Special education should be started from university level and should be continued in the form of in-service training. It is clear that these efforts are going to make remarkable increase of positive attitudes towards organ donation in Iran.},

title = {The Study of Type and Amount of Medication Errors in Neonatal Intensive Care Units and Neonatal units}, 
abstract ={Abstract Aims: One of the common medical errors is medication errors. Since medication errors are doubly important in neonatal units and neonatal intensive care units, prevention of these errors are very important and necessary. The aim of this study has been determining the level and the type of medication errors in neonatal units and neonatal care units. Methods: The present research was a descriptive-correlative study. The method of sampling was census and included 119 nurses working in the neonatal units and neonatal intensive care units in 5 educational hospitals of ShahidBeheshti University of Medical Sciences. The data collection tool was questionnaire. The first questionnaire contained the demographic data. The second questionnaire was “ Medication Administration Error” in order to determine the level and types of medication errors) and they were used after that their validity and reliability were examined. The collected data were analyzed by SPSS 18 software. Results: 11.8 percent of the nurses indicated that during the last 6 months they did not commit medication errors and 37.8 percent of them had between 1 to 2 medication errors. Medication errors in injectable drugs respectively included error in time of administration of medication (51 to 60 percent), errors in pharmaceutical calculations (51 to 60 percent), neglecting the drug interaction in simultaneous prescription of drugs (51 to 60 percent), wrong dose of a drug (41 to 50 percent) and medication errors in non-injectable medications included respectively in medication calculations (51 to 60 percent) and after that error in medication dose (41 to 50 percent). Correlation between injectable and non-injectable medication showed that error is mostly in injectable drugs and nurses who have error in injectable medication also have errors in non-injectable medications. Conclusion: Identification of types and level of medication errors can lead to steps towards prevention of these errors and advancement in the increase of quality of neonatal care and safety.},

title = {Assessment of the effect of patient’s education and telephone follow up by nurse on readmissions of the patients with heart failure}, 
abstract ={Abstract Aims: This study has been done with the aim of assessing the effect of patient’s education and telephone follow up by nurse on readmissions of the patients with heart failure. Method: In this experimental study that had been done in 2011, 189 heart failure patients hospitalized in ShahidRajayee cardiovascular hospital were selected by purposive sampling method and they were randomly divided into 3 control or intervention groups . The control group received only hospital routine care. The first intervention group received one hour face to face education and one educational researcher made booklet at the time of discharge. Second intervention group in addition to receiving one hour face to face education and educational booklet at the time of discharge, received telephone follow up by nurse for the next 3 months. Data collection tools included demographic questionnaire and readmission check lists. Data were analyzed by statistical tests and spss16 software. Results: patient’s education during discharge and telephone follow up after discharge decrease readmission level significantly. After 3 months of follow-up there was significant difference respectively for hospital, clinical and doctor’s office readmissions (p=0.06), (p=000) (p=000) between groups. Conclusion: Patients’ rapid follow up after discharge can be suggested as away in order to decrease readmissions.},

Methods : It was a semi experimental study with the method of before-after, which has been done on 86 hemodialysis patients with a Purposivesampling method within 3 months in 2011 in 1 and 2 chosen hospitals. The intervention was an intravenous injection of 100mcg/ml of vitamin B 12 afterdialysis process which has been done 3 times a week, within 2 months and by the end of the intervention in ordertoassess sustainability of theeffect ofvitaminB12 there was no interventions for 1 month. Fatigue measurementquestionnairewas completedbyparticipants and it was analyzed by using SPSS software and descriptive statistic tests, paired t-test and analysis-variance of the repetitive measures. Results: Fatigueafter a month ofinterfering (P=0.001)andwith further reductionafter twomonths (p=0.0001)andone monthafterthe end of intervention (P=0.021) has been significantlyreduced. Conclusions: The resultsindicatethe effectiveness ofvitaminB12on the reduction of fatigue in hemodialysis patients. Thus intravenous injection of 100mcg/ml of Vitamin B12 weekly after dialysis is recommended for HD patients.},

abstract ={Abstract Aims: the ultimate aim in myocardial infarction is exact monitoring and minimizing its aspects such as blood pressure that may be under the effect of body position in measuring time. The present study had been done with the aim of determining the effect of change in body condition on blood pressure of the patients with acute myocardial infarction hospitalized in CCU. Methods : This study is an experimental study that 85 patients who suffer from acute myocardial infarction had been chosen with the available method from 22nd of Bahman hospital of Gonabad in 2012. Patients for 15 minutes were in supine sleeping position, in the Right and left Lateral, Semi-Fowler’s and Fowler’s position and blood pressure had been measured in every one of these positions. Data had been measured with the version 14 of SPSS software and nonparametric tests (Friedman tests) and descriptive statistic including: indicators of central tendency and distribution. Amounts of P less than 0.05 had been considered significant. Results: Data analysis showed significant difference between blood pressure amounts in different positions. In a way that right hand and left hand systolic and diastolic blood pressures in sitting position had the most average and in the right-side sleeping position had the least average. Conclusion: According to the importance of regular blood pressure monitoring in these patients, choosing appropriate body position for determining real blood pressure and to be informed of its changes in different positions of body in patients who suffer from myocardial infarction is very essential.},

abstract ={Abstract Aims : hospitalization in CCU is always along with stress for the patients. As stress in healthy people can cause increase in heartbeat, cardiac output and blood pressure also in heart patients it can effect on physiologic factors and increase in death rate in these patients. According to the effect of emotional ties and increase of relationship with the relatives on decrease of these patients’ stress, this study had been done with the aim of determining the effect of planned meetings on physiologic indicators of the patients with ACS. Methods: This is a semi-experimental study that had been done on 72 ACS patients in CCU. Samples had been chosen with base on target method sampling and had been studied randomly in two intervention and control groups in two CCUs separately. Meeting plan in intervention group was as scheduled and included three times meeting in the second and third day of hospitalization with the presence of one to three relatives that the patient had requested and in control group it was in the form of one time meeting per day and with the presence of some open meetings. Patients’ Blood pressure, heartbeat, breathing and heart dysrhythmias were recorded every two hours in day and night and also before, after and during meeting. Results: The results show significant decrease in physiologic indicators in the third day of hospitalization in compare with the first day in intervention group in contrast with control group, also the above variables in intervention group during meeting and after meeting were significantly less than control group. (P&#60;; 0.05) Conclusion: Results of this study indicate that supportive effect of planed meeting on the patients is more than limited meeting and because of this it causes decrease of physiologic indicators and consequently it helps treatment quality improvement of the patients with ACS in CCU.},

The aim of this study is comparing the effect of follow-up care by telephone and short massage services on patient’s quality of life after cardiac valve replacement surgery . Methods: This clinical trial study was done on patients admitted for valve replacement surgery in Tehran selected hospitals in 2011.99 samples selected with based on target method and distributed according to random allocation method to telephone follow-up, short message services follow-up and control groups. In this study patient’s quality of life assessed by Nottingham quality of life questionnaire before and 3 months after surgery . Results: The mean scores of aspects of life quality in telephone follow-up, short message services follow-up and control groups decreased after intervention (that shows the better quality of life) but Anova test did not show significant difference between the mean scores of aspects of life quality in the groups before and after intervention. Conclusion : There was no significant difference between follow-up care by telephone and short massage services on patients’ quality of life after cardiac valve replacement surgery. We hope to see more usage of easy and low cost follow up care methods in cardiac care centers.},

abstract ={Aims: Transition is­ a passage or movement from one stage of life, condition, or place to another. Chronic patients somehow experience transition and nurses have an important role in supporting them. This study was aimed to explore the meaning of transition to hemodialysis. Methods: In this qualitative research, hemodialysis purposeful sampling and then theoretical sampling had been continued until data saturation (13 people). Participants were hemodialysis patients that were referring to affiliated hospitals with Shahid Beheshti University of Medical Sciences and persons being in direct and close touch with them in 2012. Semi-structured interviews were undertaken and field notes and memos were used for data collection. The data were analyzed by content analysis. Data rigor was confirmed by Lincoln &#38; Goba criteria. Ethical considerations were observed. Results: The participants showed dual meanings of transition to hemodialysis (positive &#38; negative). Categories of recovery &#38; healing way, an opportunity for interaction, an opportunity for learning, a hope point, God’s gift, and renewal life demonstrated positive meanings of transition to hemodialysis. Categories of valueless of hemodialysis, useless living with hemodialysis, life bitterness with hemodialysis, and hardship of living with hemodialysis and tragedy of life with hemodialysis, demonstrated negative meanings of transition to hemodialysis. Conclusions: Considering complexity and multidimensionality of transition and influences of means associated with transition and formation of intensity and nature of the consequences that clients experience, must be relied on reinforcing positive meanings of transition to hemodialysis in nursing care.},

abstract ={Aims: Illness perception plays an important role in se lf-care behavior and treatment adherence in hemodialysis patients. So interventions that promote illness perception can cause improvement in treatment adherence of Hemodialysis patients. Hence, this study had been conducted with the aim of determining the effect of illness perception promoting interventions on treatment adherence in hemodialysis patients. Methods: In this randomized clinical trial, 71 patients undergoing hemodialysis were allocated randomly to the intervention (n=38) and control (n=33) groups. For data collection, a two-part questionnaire consists of socio-demographic questionnaire and questionnaire of treatment adherence of hemodialysis patients (ESRD-AQ) were used. After pretest, patients in intervention group received illness perception promoting interventions individually for six sessions and control group just received routine care of the unit. Posttest was done 8 weeks after the end of intervention. The data of the study were analyzed by using SPSS-16 software and independent and paired T test. Results: Results showed that patients participating in illness perception promoting interventions had significantly higher scores in the areas of adherence to medication (p=0.01), adherence to liquid limitation (p=0.001), adherence to dietary restriction (p=0.001) and shortening durance of the hemodialysis sessions (p= 0.04) than patients in control group. Conclusions: Illness perception promoting interventions causes improvement in some dimensions of treatment adherence in hemodialysis patients. Considering inadequate research literature, there is a need for more research in this area.},

title = {Effectiveness of Muscle Relaxation on Pain, Pruritus and Vital Signs of Patients with Burns}, 
abstract ={Aims: Pain is the main complain of hospitalized patients suffering from burns. Anxiety, troubled interaction with others, disturbed sleep, movement defect, loss of appetite, low quality of life, and increase in healthcare and hospitalization costs are the consequences of non-alleviated pain and pruritus. Regarding this the present study aimed to investigate the effectiveness of muscle relaxation on severity of pain, pruritus and vital signs among patients suffering from burns. Methods: In a semi-experiment study and by using a simple convenient sampling method, 110 hospitalized patients of Burn Ward were randomly chosen in two separated stages and allocated to case and control groups. It took two consecutive months to choose the controls and cases respectively. Tools for data gathering were as follows demographic information, a pain visual analogue scale, and pruritus assessment scale and documentation form (vital signs, pruritus, pain) . As the desired intervention, cases were delivered a twenty-minute Benson muscle relaxation and then their pain, pruritus and vital signs were measured and compared to these in controls. Achieved data were analyzed by SPSS software. Results: There were no significant differences between severity of pain (mean of pain in controls and cases were 5.8 and 6.3 respectively), itching (mean of pruritus in controls and cases were 3.5 and 2.6 respectively), and vital signs (respiration, pulse rate, blood pressure, perspiration and dilated pupils) in two groups (p&#62;0.05). But, conversely, a post-intervention independent t-test revealed that there were significant differences between severity of pain (mean of pain in controls and cases were 5.5 and 2.2 respectively), itching (mean of pruritus in controls and cases were 3.6 and 2.1 respectively) and vital signs (respiration, pulse, perspiration and dilated pupils) in two compared groups (p&#60;; 0.05). However, blood pressure showed no significant difference between two groups after intervention. Conclusions: Muscle relaxation technique can be effective in relieving the pain, pruritus and vital signs of patients suffering from burns. Therefore, its use as a therapeutic and alleviative method could be suggested for patients suffering from burns .},

abstract ={Aims: Due to increasing the number of Hemodialysis patients and also the necessity of consistent and continuous cares by family members to patients, we have been up to doing a test in order to find out the effects of oral Vitamin C on tiredness and exhaustion of hemodialysis patient caregivers according to researches that have been done on the effect of oral Vitamin C on fatigue. Methods: This research is a before-after one-group semi experimental study of a that has been done in 2012. The samples were 25 caregivers who have been coming to the selected hospital in Tehran and they had been chosen randomly and purposively. At first The Multidimensional Fatigue Inventory MFI-20 Questionnaire was completed by research samples before intervention and after a month of using oral Vitamin C the Questionnaire was completed once again. After collecting the data by descriptive statistics and paired t-test the data had been analyzed by SPSS 17 software. Results: The samples were 48% male with the age average of 44.4 and 72% married. In general exhaustion and fatigue, physical fatigue, mental fatigue and reduced activity dimensions significant difference (p&#60;; 0.05) had been observed but there was no significant difference in the reduced motivation dimension. Conclusions: According to the results, using oral Vitamin C can be recommended for reducing the fatigue of caregivers and thereupon increasing the quality of care given to Hemodialysis patients.},

This study had been done with the aim of assessing the effect of healthy belief model application on health behaviors of patients with Myocardial Infarction. Methods : In this quasi-experimental research, 74 samples had been chosen by purposeful sampling method they were in two experimental and control groups in Kermanshah in 2011 . Experimental group achieved education based on health belief model for health behaviors and control group achieved traditional education. The data were gathered by a questionnaire with reliability of r=0.81 and collection checklist and statistical analysis with SPSS-18 software and descriptive and inferential statistical tests. Results: The averages age of the two groups were the same and most of the samples were males. There was signification difference between model concepts in both groups in the stage after interference in all elements except nutritional function (p=0.001). Conclusions: In order to increase knowledge and improvement of activity function of the patients with Myocardial Infarction, it is recommended to use health belief model but education with this model did not have any tremendous impact on nutritional function.},

abstract ={Aims: According to this approach that: “standardization of nursing care according to the last evidences has the best results in increasing care quality”, the aim of this study has been determined: “Designing evidence based nursing care instructions for mechanically ventilated patients”. Methods: The study had been done with “developmental research” method with the approach based on evidences in Intensive Care Units of Baqiyatallah hospital in 1392. At first quality of 15 present instructions in Intensive Care Units had been evaluated in three levels of weak, moderate and good. Then 22 new instructions had been designed according to Stetler model and with the approach based on evidences. For assessing content validity of the instructions, opinions of ten faculty members of Baqiyatallah, Tehran and Shahid Beheshti Universities had been considered in three refers by Delfi method. Applicability of the instructions had been assessed by focused group discussion method with experienced experts of Intensive Care Units. Results: Evaluation of the 15 present instructions showed that their quality is in the medium. In addition to reviewing nursing books, 655 articles had been studied, and with considering criteria of choosing evidences, 121 articles had been chosen as credible evidences. At the end 22 care instructions with good quality had been designed. Conclusions: Lack of specification of the present instructions and their low quality showed necessity of designing instructions based on evidences with high quality and specifically for different units. Designing such instructions prepares background of using nursing studies at bedside and it is going to be an effective step for based on evidence nursing. So it is recommend that designing such application instructions should be in the priority of the next studies.},

abstract ={Aims: Level of quality of life in hemodialysis patients change following stressful factors. Patients use different coping mechanisms in order to deal with stressful factors. The aim of this study was to determine stressful factors, coping mechanisms and quality of life among hemodialysis patients. Methods: This descriptive analytic study was done on 113 hemodialysis patients in Babol’s Shahid Beheshti hospital in 2010-2011. The data collection tools included questionnaire of hemodialysis stressful factors information (HSS), Jalowice &#38; Powers coping methods and quality of life Index of Parila et al. in order to analyze data, descriptive analytical statistics (Independent T - test, variance and Pearson s analysis and SPSS 17 software had been used. Results : 50.4 % of the patients were male. The age range of the patients was 87-18. The results showed that there was significant relationship between age with physiological stressful factors (p = 0.050) and there was significant relationship between residency and psychological stressful factors) and between important activities of life and variables of occupation and marital status (p=0.000 both). In dealing with stressful factors patients mostly used emotion-oriented coping methods, and there was positive and linearly relationship between emotion-oriented coping methods with psycho and social stressful factors (p=0.000 and p=0.009). There was positive and linear relationship between problem–oriented coping methods with aspects of overall quality of life and also there was significant inverse relationship between emotion-oriented coping methods with important activities of life, overall quality of life and physical condition. Conclusions : By identifying the stressful factors in hemodialysis patients, it is possible to have more valuable step through more effective nursing services and producing basic programs such as psychology counseling for strengthening problem - oriented coping mechanisms and improving their quality of life .},

Methods : This is a descriptive – analytical study, convenience sampling had been done including 800 patients that were hospitalized by diagnosis of ACS in CCU of Jahrom Medical science University hospitals in 2012. In order to collect data “assessment of patients’ characteristics based on synergy model” checklist with “inquiry” method had been used. In order to analyze data SPSS 16 software and descriptive and inferential statistical methods and Kendal and Spearman correlation coefficients had been used. Results : In the samples of the study 50.5% were male. The average age of the patients was 61.20±1.26. Findings showed that the maximum frequency of the patients considering vulnerability and complexity in level 1, in the case of stability and partnership in taking decision in level 3 and in the characteristics of returning, sources, partnership in taking care and anticipation, was in level 5. Conclusions : Hospitalized patients suffering from ACS includes a wide range in severity of disease, clinical signs and individual and social conditions. Therefore, categorization based on different conditions for providing appropriate decision and strategy aimed at gaining maximum benefit is necessary for the patient. Using synergy model leads to predicting care needs of the patients.},

abstract ={Aims : Fatigue is one of the most common dialysis side effects and it has been among the main nursing diagnosis in hemodialysis patients and it can predict cardiovascular events and its mortality in hemodialysis patients as a bioalarm. Considering that decrease in Serum level of vitamin C is one of the main causes of fatigue in these patients, this study had been done with the aim of evaluating the effect of supplementation with intravenous vitamin C on fatigue in hemodialysis patients . Methods : In a randomized, parallel group, double-blind clinical trial , 177 patients with chronic renal failure who referred to hemodialysis units of Baqiatallah and Chamran hospital in Tehran were studied. They distributed in three groups of experimental, control and placebo in 2011. In addition to demographic questionnaire , multidimensional questionnaire (MFI) had also been used for determining fatigue before and after intervention. Then in experimental group an 2.5cc injection contacting 250 mg vitamin C was injected three times every week, at the end of each dialysis session via the intravenous route , for 8 weeks. In the placebo group normal saline placebo had been injected with the same amount for the same time and there was no intervention in the control group. In order to analyze data statistically and compare them between groups, SPSS statistical software , and Chi square, T-test, Paired t-test and ANOVA were used . Results: Complain of fatigue was observed in 81/6 % of patients and 30/7% of these patients suffered from severe fatigue before the intervention . The average loss of activity, physical fatigue and general fatigue had respectively higher mean in compare with the other five of fatigue dimensions .The mean of total scores of fatigue five dimensions was 65.39±15.92. After the end of intervention there was significant difference between findings of the three groups fatigue mean was decreased in experimental group to ( 52.28±13.82). Conclusion: Vitamin C supplementation can decrease fatigue significantly and it can be used as a simple, safe and inexpensive way .},

Applicability of the instructions had been assessed by focused group discussion method with 10 experienced experts of Mental Units. Results: In addition to reviewing nursing books, 100 articles from 2006-2013 had been studied, and with considering criteria of choosing evidences, 40 articles had been chosen as credible evidences. At the end, 7 evidenced -based nursing care instructions had been designed for the patients with psychiatric Emergencies. Conclusions: With designing Evidence-based nursing care instructions in psychiatric emergencies, some needs of clinical nurses in confronting these patients were removed . So it is recommend performing such instructions and to design and perform care instructions in other units in the next studies.},

The aim of this study was nurses’ understanding of IT acceptance according to IT acceptance model. Methods : It was a descriptive-analytic study.47 nurses, working in ICU were chosen through available method from three chosen hospitals of Tehran in 2012. Data collection tool was a questionnaire based on IT acceptance model and planned behavior theory by considering reliance variable. Relationship between the main construct of the model with Spearman test and simple regression test had been used for predicting relationship between IT acceptance model constructs. Data were analyzed by SPSS16 software and descriptive and inferential statistical tests. Findings: there was significant relationship (P=0.02) between nurses’ education level with their reliance on IT. There was strong correlation between reliance and understanding ease of use. There wasn’t any correlation between attitude constructs, behavioral control and subjective norms with nurses’ trend to use IT. Behavioral trend to use IT is remarkably influenced by understanding ease of use. Conclusion : educational planners and IT managers can promote ICU nurses trend to use technology by strengthening constructs of perceiving ease of use, subjective norms and reliance},

The aim of this study was to determinate the effect of educational software of dysrhythmia simulator on ECG interpretations knowledge among nurses who are working in intensive care units. Methods : This quasi-experimental study conducted with participation of 73 ICU and CCU nurses from two Seyedoshohada and Taleghani hospitals of Oroumiyeh that were randomly placed in two groups intervention and control . Data were gathered by a researcher-made questionnaire, which verified their validity and reliability and was consisted of 15 theoretical questions and 25 dysrhythmia strips in the form of pretest-posttest design. The collected data were analyzed by SPSS statistical software and descriptive statistics (ver.16). Results: The mean and standard deviation of the nurses’ before and after scores, the group that used educational software of arrhythmia simulator method was 17.21±2.04 and 22.15±3.27 respectively, whereas it was 16.77±4.86 and 19.13±5.28 in the conference group, respectively. The Independent statistical T-test revealed a significant statistical difference between the post training scores in two groups (p=0/004). Also the comparison of average of scores in the form of intra- group with paired t-test revealed significant differences in before and after scores in every group (p&#60;; 0/005). However, the mean difference between pre and post scores in the intervention group was higher. Conclusion: The use of educational software of dysrhythmia simulator causes more improvement of the knowledge of nurses in dysrhythmia interpretation field. According to the results of this study, the ease of using and availability of simulator software, and its application in wards can facilitate in-service training for staff, particularly nurses who are working in intensive care units and also enhance their knowledge and skills in the interpretation of electrocardiograms effectively.},

The present study had been done with the aim of “determining the amount of pharmaceutical knowledge and pharmaceutical calculations skill in CCU among nursing students of Oroumiyeh and Khouy and comparing them”. Methods : this cross-sectional study had been done on 120 nursing students of Orumiyeh and Khouy through census method in the academic year of 2013. Data collection tool was a questionnaire including three parts: demographic information, pharmacology knowledge and pharmaceutical calculations that had been analyzed after their collection through descriptive statistic and long linear (row effect) test by using SAS 92 software and chi 2 test in SPSS16 software. Results: findings showed that pharmaceutical knowledge in the area of nursing cares among nursing college students of Oroumieh 43.3% and Khouy students 48.3% was good and there was no significant difference. In the area of drug class, the amount of familiarity , Oroumieh nursing students 76.7% and Khoy students 48.3% was good andt there was significant difference (P=0.0). About pharmaceutical calculations, Oroumieh students 48.3% and Kouy students 16.7% was good and there was significant difference (P&#60;; 0.001). Conclusion: considering the results of the study, the pharmaceutical calculation power of all the nursing students is not in the appropriate level. So it is required to promote pharmaceutical knowledge and pharmaceutical calculations skills by the professors and teachers for decreasing risks and increasing patient’s safety and it needs exact planning in nursing students educational programs.},

abstract ={Aims : Few studies have assessed physical and emotional distressing symptoms in patients with chronic renal failure undergoing hemodialysis. The aim of this study was to investigate distressing symptoms and its relationship with the quality of life of the patients, suffering from chronic renal failure undergoing hemodialysis. Methods: In this cross sectional study, two hundred patients with chronic renal failure undergoing hemodialysis at four dialysis units in Karaj, in 2011 were interviewed and also their medical records were investigated. Sampling method was census sampling. The used instruments included demographic data form, dialysis symptom index and World Health Organization quality of life (WHOQOL-BREF) questionnaire .Data were analyzed by SPSS software and using descriptive statistics, t-test, ANOVA and Pearson coefficient correlation. Results : Findings showed that, the most common distressing symptoms was related to decreased interest in sex (87%), difficulty of becoming sexually aroused (81%), fatigue(68.5%) and bone or joint pain(57%) . 77% had mild and 20.5% had moderate distressing symptoms severity. There were significant positive correlation between frequency score of distressing symptoms with age (p&#60;; 0.0001) and negative and significant correlation with physical health (p&#60;; 0.0001) social relationships (p&#60;;-0.011), mental health (p&#60;; 0.0001), environmental health (p&#60;; 0.0001) and total score of quality of life (p&#60;; 0.0001). Distressing symptoms severity had significant positive correlation with age (p=-0.006) and negative correlation with physical health (p&#60;; 0.0001) social relationships (p&#60;; 0.0001), mental health (p&#60;; 0.0001), environmental health (p&#60;; 0.0001) and total score of quality of life (p&#60;; 0.0001). Conclusions: Hemodialysis patients experience many distressing symptoms that negatively affect their quality of life . Teaching self-care behaviors in order to reduce and control distressing symptoms can help to improve the quality of life in hemodialysis patients .},

abstract ={Aims: Infants, who are hospitalized, may need transportation due to different reasons that may happen in inter or intra-hospital form, but what should be noted in transferring an infant is stabilization of the infant’s situation before transportation and maintaining stability during transportation. So this study had been designed and performed with the aim of “determining intra-hospital situation”. Methods: In this cross-sectional study, which had been done in selected hospitals of Tehran Medical Sciences University in 2013, totally 52 intra-hospital infants’ transportations had been assessed by using equipment standard checklist of the least transportation, transportation team features, and the questionnaire of the infants’ situation and the checklist of performing infant’s stabilization program. Data were analyzed by using SPSS 19 software and descriptive statistic. Results: Most of the transportations had been done by one single person and by specialist nurses, most of these people were in the age range of 20-29 years old (46.6%) and they were female (72.6%) and they had the experience of working in ICU (71%). Most of them had not passed the neonatal resuscitation courses, or it was more than six months since their last passed course (61.2%). the majority of the transportations had been done without monitoring blood oxygen (56.6%). There wasn’t enough necessary equipment and drugs with the neonet during transportation in most of the transportations (71.8%). Conlusion: Intra-hospital transportation situation in the neonets is far from the international standard. Considering these conditions, it is expected that authorities of nursing permanent education to hold training and skill courses of neonet’s transportation and highlight the part of emotional support in educational programs. Also paying attention to use special incubators and having necessary equipment and drugs needed for transportation are essential.},

Methods: in and intervention study, 72 of medical emergencies staff of north of Khouzestan, who were selected randomly in 2013 were tested, before education, immediately after and 15 days after education (by a triage 45 minutes educational film and after 15 minutes asking and answering) by a researcher-made questionnaire, which included three parts of questions (demographic, awareness and performance), then results were analyzed by SPSS18 software and descriptive test and paired t-test. Results: the average of awareness and performance were respectively increased from 39% to 84f.6% and from 29.31 to 75.57 and also there was significant statistical relationship (P&#60;; 0.001). Also there was significant statistical difference between the average of awareness and performance immediately after and 15 days after education (P&#60;; 0.001).},

All cardiac valve-replaced patients need to use warfarin during their life in order to reduce the risk of valvular thrombosis . There were some reports about various problems of using warfarin by considering care conditions, but there was no comprehensive study based on patients’ experiences. The present study was carried out with the aim of “explaining the experiences of cardiac valve-replaced patients about using warfarin”. Methods: This study was conducted with a qualitative research approach and conventional content analysis in 2013. The Participants were 10 cardiac valve-replaced patients who had experiences of different time periods in Tehran and Kashan therapeutic centers and outpatients who had the experience of living with metal cardiac valves. Data collection was performed through semi-structured interview and purposive sampling and it continued until data saturation. Data analysis had been done continually and concurrently with data collection and it was performed through a comparative method. Results: 479 codes were extracted from deep and rich descriptions of the participants. There were two main themes during data analysis process including: concern and fear of changes in the International Normalized Ratio (INR) and the struggle for the correct use of warfarin and adjustment of INR , the first theme was fear and concern about warfarin use which was consisted of five subcategories concern on the correct use of warfarin concern on health cares, warfarin side effects concern on pregnancy and dental care, the second theme was effort for correct use of warfarin and regulating its use which was consisted of five subcategories: perceiving the necessity of warfarin use, adherence to the physician, self-care, physicians’ recommendations and health care and educational requirements for warfarin use. Conclusions: Cardiac valve-replaced patients have a permanent fear and concern during thetime of warfarin use and they try to reduce it with the correct use of warfarin and the adjustment of INR. It is necessary to help them to reduce such concerns through actual knowledge of the patients, providing post-discharge care and establishing the society for cardiac valve-replaced patients and the family.},

title = {The effect of Riker sedation-agitation scale on clinical outcome of patients under coronary artery bypass graft surgery}, 
abstract ={Aims: frequent investigation of the patient for determining sedation level is one of the main measures in care of critically ill patient especially after heart surgeries. Riker scale is a valid and reliable tool which has been used in different studies as sedation-agitation scale in patients hospitalized in Intensive Care Unit (ICU). This study had been done with the aim of “investigating the effect of Riker scale on clinical outcome of patients under coronary artery bypass graft surgery (CABG)” . Methods: this clinical trial study was done in Imam Ali Hospital of Kermanshah in 2012. 116 patients, after coronary artery bypass graft surgery were selected and randomly divided into equal intervention and control groups through convenience sampling. In the intervention group, the level of patient’s sedation was monitored by Riker scale with Kappa agreement coefficient r=0.92 and in control group it was done by a method based on physiologic responses. Data were collected with researcher-made questionnaire and checklist which had face and content validity and they were analyzed with SPSS-18 software and descriptive and inferential statistical tests (chi-square, independent t and fisher). Results: there was a significant reduction in drug consumption of sufentanil (as sedating drugs) (8±5.5 vs. 23±12.5 microgram (mcg)), duration of mechanical ventilation (7.1±2.37 vs. 9.3±2.6 hours) and duration of ventilation with SIMV mode (3.7 ± 1.78 vs. 5.45± 2.14 hours) in intervention group in compare with control group (P&#60;; 0/001). There wasn’t any significant difference statistically between two groups regarding length of stay in ICU and hospital and delirium appearance. Conclusion: Findings of this study suggest use of Riker sedation- agitation scale for monitoring awakening in patients undergoing coronary artery bypass graft surgery in ICUs with the aim of decreasing duration of mechanical ventilation and sedative drugs.},

abstract ={Aim : Burn pain is one of the severest and most consistent types of pain. Patients who suffer from burn usually experience the highest level of pain at the time of dressing change. The aim of the present study is “determining the effect of deep-slow and regular breathing on pain intensity of dressing in patients with burns.” Methods: This study is a randomized clinical trial in two intervention and control groups which investigate the pain in 68 patients referring to the burn ward of Kashani hospital of Shahrekord in the period of March 2011 to August 2011 through random and convenient sampling, these patients were in one of intervention or control group. Data were collected with visual analogue scale and analyzed by using SPSS, descriptive statistic test, independent t and chi - square. Results : At first the mean of dressing pain intensity score before and after intervention in intervention group was 5/82, 3/66 and in control group was 5/3, 4/8 respectively. There was significant difference between the pain intensity after dressing in intervention and control groups (p=0.04). Conclusion : Using deep-slow and regular breathing can be an important factor in reducing pain intensity during burn dressing.},

This study was done with the aim of “comparing the effect of metatarsus and ankle reflexology massage on patients’ anxiety. Methods : In this clinical trial, the research society included men and women undergoing Coronary Artery Bypass Graft Surgery in Imam Ali hospital of Kermanshah that were randomly assigned to one of the three groups of metatarsus , ankle reflexology massage and control groups in 2012 . Anxiety is measured through Spileburge’s State Anxiety Standard questionnaire SPSS16 software, ANOVA and Mann Whitney were used for data analysis. Result : Anxiety was decreased after using metatarsus and ankle reflexology massage. There was significant difference between state anxiety mean scores in control group and metatarsus reflexology massage group and also between control group and ankle reflexology massage group in the first day (p&#60;; 0.001).. Conclusion : Metatarsus and ankle reflexology massage are effective in decreasing state anxiety in patients undergoing coronary artery bypass graft surgery. Therefore it is recommended to use these complementary methods for decreasing anxiety.},

abstract ={Aims: although artery coronary angiography is the most important method of diagnosing cardiovascular diseases, it is an important factor in increasing patients’ anxiety. Considering that there are different medicinal and non-medicinal methods to decrease anxiety, this paper tried to study the effect of foot reflexology on the anxiety of patients undergoing artery coronary angiography. Methods: Seventy patients undergoing artery coronary angiography in Valiasr hospital Affiliated with Medical Sciences University of Birjand were assessed in 2013. They were selected through convenience sampling and divided randomly into two intervention and control groups. Spielberger state-trait inventory was used to collect data. Any patient in the intervention group received foot reflexology for 20 minutes and patients in the control group received no reflexology. The extent to which patients felt anxious in the two groups before, immediately after, and half an hour after the intervention was measured, and the data were analyzed in SPSS 16 software by Chi-square and independent t test. Results: There was a significant difference between the two groups anxiety scores mean before, immediately after, and half an hour after the intervention (p=0.096) also there was significant difference between changes in anxiety scores mean before, immediately after, and half an hour after the intervention in control and intervention groups(p&#60;; 0.001). Conclusion: The results of this study regarding the effect of foot reflexology massage on anxiety decrease of patients undergoing coronary angiography, in the case of results conformity in larger studies, can be used as a safe, effective, applicable and affordable intervention in decreasing patients anxiety in health care centers and hospitals.},

abstract ={Aims: One of the main duties of treatment team in ICU is relieving pain of the patients hospitalized in this ward. Using pain protocols can have positive effects in reducing pain experience of patients hospitalized in ICU. The present study has been done with the aim of “investigating the effect of applying the protocols of pain and sedation control of the patients hospitalized in intensive care unit.” Methods: In a semi experimental study in Khatamolanbia hospital of Zahedan in 2012 , we surveyed 80 critically ill patients who were under mechanical ventilation. We randomly assigned patients in two control and intervention groups while in intervention group, patients’ pain and sedation control were performed by using pain and sedation protocol in control group, patients received usual, none protocol pain and sedation control. For data collection, behavioral pain scale (BPS) and Richmond Agitation Sedation Scale (RASS) were used. After data collection, data were analyzed by SPSS 18 software and by using descriptive and inferential statistical tests. Results: there wasn’t any significant difference in terms of age and sex between two groups (p&#60;; 0.264). Level of pain of patients in intervention group (protocol group) was significantly lower in compare with patients in control group (p&#60;; 0.000). Also, level of patient’s sedation in intervention group was near to ideal sedation of Richmond in compare with patients in control group. There was statistical significant difference in sedation level of patients between the two groups (p=0.05). Conclusion: Many patients in ICU experience pain as an unpleasant experience. Results of the present study showed that using pain and sedation protocol can improve pain control and also better sedation remarkably in patients hospitalized in ICU.},

Methods : This descriptive - analytical comparison study was done on 100 first-degree family members of the patients hospitalized in ICU or CCU of Khatam-ol- Anbiya hospital in Zahedan in 2011 . Data were collected by the questionnaire of surveying psychosocial needs of the patients hospitalized in ICUs and they were analyzed by using SPSS 18 software, parametric t-tests and Fisher’s exact test. Results : there was significant statistical difference between two groups in some items such as need to meeting and changing in its times , nurses accepted the family members, talking to them about their feelings, understanding about ICU and participating in patient’s treatment (P&#60;; 0.05). Conclusion: considering the significance of these items into two groups , it seems that creating more opportunities for meeting and also giving more information to the family members about patients’ treatment process help to improve the psychosocial needs and reduce Psychological reactions caused by the patients’ family members in ICU},

title = {Factors affecting triage decision-making from the viewpoints of emergency ward staff in Tabriz hospitals}, 
abstract ={Aims : Triage is the most important measures in the emergency ward. Accurate triage decision has a significant impact on patients’ outcomes. The aim of the present study was to investigate the factors influencing triage decision making in the emergency wards . Methods: All physicians (N=22) and nurses (N= 135) in 18 emergency wards of Tabriz hospitals participated in this descriptive study. For data collection a questionnaire was used that assessed staff and non-staff related factors (including patients and emergency ward related factors) that influencing triage decision making. Social- Demographic and professional information of staff was assessed by a checklist. Data analysis was performed by SPSS statistical software and descriptive statistics. Results: The most important factors in staff related factors were experience and skill in non-staff factors related to patient vital signs and type of injury and in non-staff factors related to ward were overcrowding and probability of injury to the patient. Also, it was found that the basis of decision-taking regarding staff and non- staff factors related to patient was correct, but this basis regarding none-staff factors related to the ward was incorrect. Conclusion: The results showed that the quality of triage decision-making among staff working in emergency wards can be improved by enhancing the quality of clinical wards’ structure.},
