abstract ={Aims. Sedative medications are widely used in intensive care unit patients. Structured assessment of sedation and agitation is useful to titrate sedative medications and to evaluate agitated behavior with persian version of Richmond agitation/sedation scale. Methods. At first the english version of Richmond translated to persian and the initial persian version made by expert confirm. This version studied on 35 patients. Then the persian version translated back to english and evaluated in 35 other patients. After teaching to users (one anesthesiologist and 7 nurses), this scale studied in 2 phases on 120 adult ICU patients in one of the Tehran hospitals to measure the inter-rater reliability and validity. Results. Comparing the intraclass correlation coefficient (RASS κ) equal to 0.65 with visual analog scale (r) equal to 0.76 and interclass correlation coefficient (α) equal to 0.95 showed a significant correlation between the raters. Conclusion. Persian version of Richmond agitation/sedation scale is logical, easy to administer and readily recalled. It has high reliability and validity in medical and surgical, ventilated or no ventilated and sedated or no sedated adult ICU patients.},

abstract ={Aims: Non-compliance of cardiac patients is one of the most challenges in controlling the cardiac diseases and readmission in cardiac care units. This study was accomplished to explore the compliance concept and effective factors on occurring non-compliance in readmitted patients with congestive heart failure (CHF). Methods: Using qualitative research, 42 CHF patients with readmission during 6 months (November 2007 until April 2008) were interviewed in two educational hospitals of Tehran University of Medical Sciences. The data were analyzed based on content analysis method. Results: Doing any unscientific action for establishing "well feeling" is the meaning of compliance in patients whereby the patients applied it as a strategic variable for resolving their main concern "running about providing comfort in daily living". Primary codes revealed three themes of "preventive factors related to the patient and his/her family", "preventive factors related to medicament and medical team" and "preventive factors related to socioeconomic problems as effective factors on occurring incompliance" which were the main causes of non-compliance in CHF patients. Conclusion: By applying qualitative research, the compliance meaning and the non-compliance predisposing factors extracts more completely. Exploring the meaning of compliance and effective factors on non-compliance in patients should be considered before discharging to prevent hospital readmissions.},

abstract ={Aims: Many patients need special care, and endotracheal tube and the use of mechanical ventilation is essential. This type of airway leads to mucosa stimulation and production of mucus. Suctioning is accomplished with complications such as severe hypoxia and significant cardiovascular disorders selecting the least dangerous way of endotracheal tube suction can reduce severe complications. This study compared the effect of two open and closed methods of suction on the pattern of heart rate and arterial blood oxygen saturation. Methods: In this cross over clinical trial study performed during 1386-87 in critical care units of Artesh hospitals, 60 patients who were connected to the ventilator were selected. Open and closed suction techniques carried on samples and heart rate patterns and arterial blood oxygen saturation was measured in patients before and several times after each type of suction and was compared. For analysis of data the paired T-test and ANOVA and SPSS 17 software was used. Results: There was a significant different on heart rate and oxygen saturation in different times after closed suction system and open suction system (p< 0.0001). These differences in different periods after the open suction method was more visible than closed but there was no change in cardiac rhythm in both open and closed suction methods (p>0.05). Conclusi on: Open and closed suction methods are effective on heart rate and arterial blood oxygen saturation and it is suspected that closed suction method makes less changes in patient hemodynamic status.},
