title = {The Relationship of Discomforting Factors with Coping Strategies
Among Patients in Cardiac Surgery Intensive Care Units},
abstract ={Background: Patients in cardiac surgery intensive care units are affected by different discomforting factors and use different strategies for their management. Objective: The aim of this study was to examine the relationship of discomforting factors with coping strategies among patients hospitalized in cardiac surgery intensive care units. Methods: This descriptive - correlational study was done in 2016. A convenience sample of 110 patients was drawn from the cardiac surgery intensive care unit of Amir - al - momenin heart surgery center, Kordkuy, Iran. Data were collected using a demographic questionnaire, the Discomforting Factors in Cardiac Surgery Intensive Care Unit Questionnaire, and Jalowiec Coping Scale. Pearson correlation analysis, the one - way analysis of variance, the paired - sample t - test and the Tukey’s post hoc test were used to analyze the data. Data analysis was performed via the SPSS software (v. 16.0) and at a significance level of less than 0.05. Results: The mean score of discomforting factors was greater than the moderate level (74.08 ± 16.93). Participants used emotion - focused coping strategies more than problem-focused ones. Discomforting factors were inversely correlated with problem - focused strategies (r = - 0.266; P = 0.005) and directly correlated with emotion-focused strategies (r = 0.247; P = 0.009). Conclusions: Patients in cardiac surgery intensive care units suffer from the negative effects of different discomforting factors and use emotion - focused coping strategies more than problem - focused strategies for the management of the factors. abstract ={Background: In obstetrics, pregnancy and childbirth are the basis for psychological and biological crises and are associated with a high prevalence of anxiety disorders. This study aimed to investigate the effectiveness of group psychological counseling on the level of pregnancy anxiety and fear of vaginal delivery in pregnant women. Methods: This Quasi-experimental study was done in 2016 on 96 pregnant women (selected via multistage cluster sampling) that had low to medium levels of anxiety and had an average of 28 or higher fear scores from birth labor. The intervention group treated with 5 sessions of group psychological counseling pre-test and post-test were completed by each of the two groups. Independent and paired t-tests were used for analyzing. Results: Results showed that the psychological counseling could reduce situational and trait anxiety in the intervention group (P < 0.001). Inthecontrolgroup,trait(P=0.007)andsituationalanxiety(P=0.004)wereincreased. Thedifferenceof situationalandtrait anxiety between two groups was significant (P < 0.001). Regarding the fear of childbirth in the intervention group, psychological counseling reduced fear of childbirth significantly (P < 0.001). Fear of childbirth showed a slight increase in the control group, and it difference score in two groups was significant (P < 0.001). Conclusions: According to results, psychological counseling was effective in reducing pregnancy anxiety and fear of childbirth. Therefore, it is recommended that the findings were considered by nurses and other medical staffs to improve the health status of pregnant women. title = {Application of Empowerment Program on Self-Efficacy of Patients
Under Hemodialysis: A Clinical Randomized Controlled Trial},
abstract ={Background: End-stage renal patients conformed to a variety of physical and psychological problems. Self-efficacy is regarded as an essential component to achieve a desirable condition in these patients. The aim of this study was determined the effectiveness of empowerment program based on the BASNEF model on the self-efficacy of patients under hemodialysis. Methods: The present study was a randomized, single-blind, parallel, controlled clinical trial that was performed on 60 patients who referred to the Hemodialysis Centre of Kashan University of Medical Sciences, Iran. Participants were assigned to two groups based on block randomization allocation. The intervention group (n = 30) received 8 sessions of empowerment program and the control group (n = 30) received the routine cares for 4 weeks. Data were collected by a Sherer’s general self-efficacy questionnaire at baseline and end of study and analyzed using Chi-square, independent and paired t-test in SPSS 13v. Results: The study was completed by 58 participants. However, as the analysis was based on an intention-to treat approach, all 60 patients were included in the final analysis. Comparison of the two groups showed that the total scores of the self-efficacy were increased after intervention (65.86 ± 16.42 vs 50.3 ± 18.02, t = 3.24, P < 0.002). In addition, significant difference was observed between the mean score of self-efficacy in the intervention group prior to and after the empowerment program (P < 0.0001), while no significant change was observed in the control group, before and after the study (P = 0.17). No harms and side effects associated with intervention were in the groups. Conclusions: The empowerment program was effective on self-efficacy of patients with end-stage renal diseases undergoing hemodialysis. Therefore, it is recommended that this program be used to increase self-efficacy in medical settings. Keywords: Behavioral Model, Empowerment, Self-Efficacy, Hemodialysis},

title = {The Effect of Training Based on the Self-Efficacy Model on the Quality of Life of Patients with Stroke},
abstract ={Background: Stroke is one of the major causes of inability in adults and the elderly. The chronic complications and the problems resulting from this disease have significant effects on the quality of life of these patients and result in diminished self-efficacy of the affected individuals. The present research was conducted with the aim of determining the effect of self-efficacy training on the quality of life and self-efficacy of patients with stroke. Methods: This is a quasi-experimental study with a control group. The study population included ischemic stroke patients among which, a sample of 80 people were chosen according to the inclusion criteria through a convenience sampling method. They were then assigned randomly into intervention and control groups (40 subjects each). For the intervention group, a training program based on the self-efficacy model was presented in five sessions, but in the control group, only routine hospital cares were provided. In order to collect data, the stroke-specific quality of life scale, whose validity and reliability were measured and confirmed, was used. The data were analyzed by SPSS 16 at descriptive and analytical statistical levels (independent t test, paired t test, and analysis of variance). Results: The findings indicated that there was no significant difference in the quality of life between the two groups before the training (P > 0.005). However, following the training, a significant difference was observed in the quality of life of patients with stroke between the intervention and control groups (P < 0.005). The results of this research indicated that the quality of life and its components in stroke patients had a greater mean value following training in the intervention group than in the control group. Conclusions: Implementing self-efficacy training programs can result in the enhanced quality of life and self-efficacy among patients with stroke. Since the promotion of self-efficacy in patients with chronic disease is important, it is suggested that educational programs are also held for other patients with chronic diseases. title = {Comparison of Enteral and Parenteral Nutrition in Patients Admitted to the ICU: A Narrative Review Study},
abstract ={Context: Nutrition support is part of the standard care in the intensive care unit. There is a paradox in the use of enteral and parenteral nutrition methods in ICU patients. Objectives: The aim of this study was to compare the effects of enteral and parenteral nutrition methods in patients admitted to the ICU. Data Sources: This article is a narrative review. A total of 619 articles, were extracted during the years of 2000 - 2018, on nutrition methods in ICU patients, with keywords enteral nutrition, parenteral nutrition, and nutrition in critical patients in databases of internal (SID, Iran Medex, Medlib) and external publications (PubMed, Scopus, Web of Science, Google Scholar); finally, 19 articles were analyzed. Results: Enteral administration reduced the associated infectious morbidity, hospitalization time, mortality, costs, non-infectious complications, multiple organ failure, systemic infections, local septic complications, and the need for surgery. It also causes early returning of intestinal movements, faster preoperative weight gain, easier fluid and electrolyte management, reduction of hyper metabolic responses, more complete nutrition, preservation of the gastrointestinal mucosa, and the ability to complete the program by the patient. Parenteral nutrition increases infectious complications, hyperglycemia, coagulation disorders and duration of hospital stay, as well as effects on invasive ventilation. Conclusions: Enteral nutrition is safer and less complicated. By improving the awareness of careers through education, patients can be improved faster. title = {Examining the Level of Self-efficacy and its Related Factors in Patients with Angina Pectoris Disease Referring to Selected Hospitals in Tehran During 2018},
abstract ={Background and Aim: Among all the different types of cardiovascular diseases, the angina pectoris disease has been considered to be the most prevalent. In order to promote the health status of those who are struggling with this disease, different factors such as self-efficacy can be considered. However, the results of previous research in regards to the factors of self-efficacy of angina pectoris patients is extremely diverse. The aim of the present study was to determine the level of self-efficacy and its related factors in patients with angina pectoris disease referring to the selected hospitals of Tehran in 2018. Methods: This cross-sectional descriptive-analytic study was performed on 270 patients with angina pectoris disease referring to the selected hospitals in Tehran who were selected using the convenient sampling method. Data collection was conducted through an interview using a two-part questioner: demographic information and self-efficacy Sullivan scale in patients with angina pectoris. The statistical analysis of data using descriptive and analytical statistics was performed in SPSS software version 20. Results: The range of self-efficacy score in patients was 0 to 52 with a mean score of 31.07±17.7. Therefore, 39% of patients had a moderate level of self-efficacy. The highest score was related to the dimension of “maintain function” (11.54±3.85) and the lowest score was related to the “control symptoms”(9.57±3.35). The results of the chi-square test revealed no significant relationship between age, sex, marital status, situation, with level of self-efficacy. This is while it showed a significant relationship between education level (P= 0.001), employment status (P= 0.001), economic situation (P= 0.007), duration of having the disease (P= 0.02), kind of disease (P= 0.009), with the level of self-efficacy in patients with angina pectoris disease. Conclusion: Considering the fact that patients with angina pectoris disease did not have high levels of self-efficacy, it is recommended that the factors affecting self-efficacy in the examined patients be further investigated. Finally, necessary steps should be taken in order to deal with these factors. Keywords: Angina Pectoris, Cardiovascular Diseases, Coronary Artery Disease, Self-efficacy},

title = {The Effectiveness of Neonatal Cardiopulmonary Resuscitation Training by Educare Method on Nurses and Midwives\' Knowledge and Performance},
abstract ={Background and Aim: Considering the importance and necessity of neonatal resuscitation training, one of the introduced methods is Educare. The Educare consists of six sections for learning skills. By using this method, the need and necessity of learning, behavioral and particular goals, practical training, supervisions, using the reminder tool, the opportunity to practice and repeat practically, and evaluating skills can be explained. Therefore, this study has aimed to investigate the effectiveness of neonatal cardiopulmonary resuscitation training by Educare method on nurses and midwives' knowledge and performance. Methods: The present study was a quasi-experiment research without a control group. It was carried out on thirty nurses and midwives of the Neonatal Intensive Care Units (NICU) and labor and maternity wards who were selected based on convenience and purpose-based sampling in Baqiyatallah Hospital in 2018. The training was carried out by conducting a two-day neonatal resuscitation workshop. The effectiveness of neonatal resuscitation training was tested in three stages of before, immediately after and two months after training by using a researcher made questionnaire and checklist after validity and reliability confirmation. The data was analyzed using descriptive statistics, paired t-test and repeated measures variance analysis by spss25 software. Results: The mean and standard deviation of knowledge and performance scores were 51.93 (SD=6/6) and -129/14(SD=34/97) before and after training, respectively. The mean and standard deviation of knowledge and performance scores were 64/53 (SD=2/36) and-25/7(SD=19/01) immediately after training respectively. Also, the mean and standard deviation of knowledge and performance scores were 59.73(SD=3/42) and -68/7(SD=28/2) two weeks after training, respectively. Findings reveal that there is a significant difference between the mean and standard deviation of knowledge and performance scores before and immediately after and two months after training (p <0.05). Knowledge and performance scores immediately increased after training compared to pre training and also two months after training. Conclusion: To conclude, it can be stated that the Educare method was effective on the level of knowledge and performance of nurses and midwives. Findings also showed that resuscitation training is extremely necessary both at regular intervals and continuously. Therefore, it is recommended that authorities and educational managers use this new method to improve the level of clinical staff’ knowledge and performance related to the neonatal at regular intervals. Keywords: Educare, Neonatal Resuscitation, Training, Knowledge, Performance},

title = {Examining the Effect of Humidifier Therapy on Hoarseness and Coughing Following Endotracheal Extubation in the Elderly after Undergoing Coronary Artery Bypass Surgery},
abstract ={Background and Aim: Hoarseness and coughing are common complications following endotracheal extubation. It has been stated that humidifier therapy has anti-inflammatory properties. Therefore, the present study has been conducted in order to determine the effect of humidifier therapy on hoarseness and coughing following endotracheal extubation in the elderly undergoing coronary artery bypass surgery. Methods: In a clinical trial, 80 elderly patients undergoing coronary artery bypass surgery were selected using convenience sampling. They were randomly divided into two control and experimental groups. The elderly patients in the experimental group were exposed to humidifier therapy after tracheal extubation. The control group received routine care. The intensity of cough and hoarseness were measured in both groups, after tracheal extubation. The severity of cough and hoarseness were recorded in both groups immediately after intervention, 6, 12, 18, and 24 hours after extubation. For analyzing the data, U Mann Whitney, chi-squared and independent t-tests were performed using SPSS version 21. Results: The effect of reducing the severity of coughing by humidifier therapy is -0.51% (P=0.001). Actually, humidifier therapy can reduce the probability of not coughing by 0.6% (P=0.001). Also, the effect of reducing hoarseness with humidifier therapy is -0.51%. Humidifier therapy can reduce the possibility of not hoarsening by 0.5%. Conclusions: The findings of the present study reveal that humidifier therapy can decrease cough and hoarseness in the elderly patients after Endotracheal Extubation. Keywords: Endotracheal Extubation, Humidifier Therapy,, Hoarseness, Cough, Elderly, Coronary Artery Bypass Surgery},

title = {Relationship between Nurses’ Subjective Workload and Occupational Cognitive Failure in Intensive Care Units},
abstract ={Background and aim: The subjective workload is a significant stressor in nursing profession and the stress can lead to cognitive problems and occupational cognitive failure. However, the relationship between subjective workload and cognitive failure is ambigious. Therefore, this study was conducted to determine the relationship between nurses’ Subjective workload and occupational cognitive failure in intensive care units. Methods: This analytical cross-sectional research was conducted on 176 nurses working in intensive care units of Rasht medical centers in 2017. Samples were selected using census method. The data collection tools consisted of personal and occupational data, standard questionnaires for subjective workload (NASA-TLX) and occupational cognitive failure (OCFQ). The data analysis was performed by the software of SPSS 21 and descriptive and inferential statistic tests of Mann-Whitney and Kruskal-Wallis, Spearman correlation. Results: The majority of samples were female (93.2%) with a mean age of 35.85 ± 7.8, married (71%) and undergraduate (92.2%), ICU (71%), Formal employment (64.2%) with an average work record of 12.21 ± 7. 06 years (86.9%). Mean score and standard deviation for subjective workload was 59.95 ± 16.41 and for occupational cognitive failure 70.31 ± 17.06. There was a significant positive correlation between occupational cognitive failure and subjective workload (r = 0.272, p <0.001). Conclusion: Considering the relationship between subjective workload and cognitive failure, the patient's safety and eventually the quality of care may be improved by designing strategies to reduce the subjective workload and considering its related factors in the planning of human resources employed in intensive care units. Keywords: Intensive care units, Nursing staff, Workload},
